Anneaux intra-cornéens Ferrara implantés au laser femtoseconde dans le traitement du kératocône : résultats fonctionnels à 1 an - 26/05/16

Introduction |
Les anneaux intra-cornéens (AIC) sont utilisés depuis plusieurs années pour la correction des astigmatismes irréguliers des patients atteints de kératocône.
Objectif |
Évaluer l’efficacité des AIC implantés par laser femtoseconde dans le traitement du kératocône et rechercher des facteurs prédictifs de succès.
Patients et méthodes |
Étude rétrospective évaluant les patients atteints de kératocône ayant bénéficié d’une implantation au laser femtoseconde d’AIC Ferrara entre décembre 2012 et mai 2014 au CHU de Besançon. Pour déterminer des facteurs prédictifs de succès, l’effectif a été divisé en deux groupes, répondeur (gain de plus de deux lignes de la meilleure acuité visuelle corrigée [MAVC] à 1 an postopératoire) versus non répondeur (gain de moins de 2 lignes de MAVC ou perte d’acuité visuelle [AV] à 1 an postopératoire). Pour chaque groupe, les valeurs réfractives et kératométriques préopératoires ont été comparées. De plus, pour vérifier si l’acuité visuelle préopératoire pouvait être un facteur prédictif de succès, 4 groupes ont été définis en fonction de l’AV de départ, le gain, la stabilité ou la perte de lignes d’AV ont été comparés.
Résultats |
Étude rétrospective portant sur 32 yeux de 29 patients atteints de kératocône ayant bénéficié d’une implantation au laser Femtoseconde d’AIC Ferrara de décembre 2012 à mai 2014 au CHU de Besançon. Dès 1 mois postopératoire et jusqu’à 1 an, il a été observé une amélioration significative de la MAVC ainsi qu’une diminution significative des valeurs cylindriques et kératométriques (p<0,05). À un an, 90,6 % des patients ont eu une augmentation de leur AV d’au moins une ligne et 9,4 % une baisse ou une stabilité de leur AV. La valeur moyenne du cylindre a diminué de −4,81±2,45 dioptries (D) en préopératoire à −2,39±1,94 D (p<0,001) à un an postopératoire. La moyenne de la kératométrie maximale est passée de 51,14±3,89 D à 48,52±4,00 D (p<0,05). Quatre facteurs prédictifs de succès, statistiquement significatifs, ont été retrouvés ; il s’agissait du type d’ectasie (ectasie type 1 ou 2), du stade du kératocône (Krumeich 1 ou 2), du nombre d’anneaux implantés (1 seul anneau) et d’une AV préopératoire inférieure à 0,4 (échelle décimale). Ces derniers gagnaient 2 lignes de MAVC dans 86 % des cas à un an et 3 lignes dans 80 % des cas contre respectivement 52 % et 41 % pour les patients avec une AV de départ ≥0,4 (échelle décimale).
Conclusion |
L’implantation d’AIC dans le traitement du kératocône est une technique fiable et réversible. L’efficacité est plus marquée pour les patients porteurs d’un kératocône asymétrique de stade moyennement avancé mais dont l’acuité visuelle de départ est supérieure à 0,4 LogMar.

Intrastromal corneal rings (ICR) have been used for several years for the correction of irregular astigmatism in patients with keratoconus.
Objective |
To evaluate the effectiveness of femtosecond-assisted ICR implantation for treatment of keratoconus and to define predictive factors of success.
Patients and methods |
We undertook a retrospective study of patients with keratoconus who underwent femtosecond-assisted Ferrara ICR implantation between December 2012 and May 2014 at the University Hospital of Besançon. To determinate predictive factors of success, we divided patients into 2 groups based on the gain of at least two lines of best corrected visual acuity (BCVA) at 1 year after surgery (“good responders”) or the gain of only one line of BCVA, no change, or the loss of lines of BCVA (“poor responders”). For each group, we analyzed preoperative refractive and keratometric values. Moreover, to verify if preoperative visual acuity could be a predictive factor of success, four groups were created based on initial visual acuity, and postoperative BCVA was compared.
Results |
We retrospectively studied 32 eyes of 29 patients with keratoconus who underwent femtosecond-assisted Ferrara ICR implantation between December 2012 and May 2014 at the University Hospital of Besançon. From one month to 1 year postoperatively, we observed a significant improvement in BCVA and a significant decrease in cylinder and keratometry values (P<0.05). In total, 90.6% of patients had an increase in visual acuity (VA) of at least one line within 1 year while 9.4% had a decrease or stability of their VA. The average cylinder value decreased from −4.81±2.45 diopters (D) preoperatively to −2.39±1.94 D (P<0.001) at 1 year postoperatively, and the average of the maximum keratometry from 51.14±3.89 D to 48.52±4.00 D (P<0.05). Four predictive factors of success were statistically significant: type of ectasia (type 1 or 2), stage of keratoconus (Krumeich 1 or 2), implantation of only 1 ICR and a preoperative VA less than 0.4 (decimal scale). Eighty-six percent of patients with an initial VA<0.4 (decimal scale) gained 2 lines of BCVA within 1 year and 80% 3 lines versus 52% and 41%, respectively for patients with initial VA≥0.4 (decimal scale).
Conclusion |
Femtosecond-assisted Ferrara ICR implantation for treatment of keratoconus is a reliable and reversible technique. Implantation results in greater efficiency for patients with moderately advanced asymmetric keratoconus with an initial VA<0.4 (decimal scale).
