Résumé |
L’arthrose du premier rayon a des conséquences fonctionnelles et algiques importantes. Site d’insertion des muscles stabilisateurs du pied à la mise en charge, cette chaîne articulaire est essentielle au bon fonctionnement du pied. Une arthrose d’un niveau de la chaîne aura des conséquences sur les autres niveaux, ainsi une arthrose de la première articulation métatarsophalangienne pourra entraîner une dégénérescence de l’interphalangienne. Le podologue intervient en première intention sur la limitation des symptômes et déficits fonctionnels, mais aussi en seconde intention en cas de chirurgie ayant entraîné une modification fonctionnelle à compenser. Quelle que soit la technique de fabrication de l’orthèse, le podologue doit répondre à quelques règles de prise en charge afin de compenser, corriger ou modifier la structure ou la fonction du pied en charge lors de la station debout ou à la marche. Un des objectifs récurrents reste la limitation de la mobilité articulaire responsable de la douleur.

Osteoarthritis of the first ray has significant consequences in terms of pain and function. The insertion point of the stabilising muscles of the foot with weight-bearing, this chain of joints is essential for the correct functioning of the foot. Osteoarthritis at some point of this chain will have consequences elsewhere. For example, osteoarthritis of the first metatarsophalangeal joint can lead to a degeneration of the interphalangeal joint. The podiatrist provides not only first-line treatment which aims to limit symptoms and functional deficits, but also second-line treatment in the case of surgery having led to a functional modification which needs to be addressed. Whatever technique is used to manufacture the orthosis, the podiatrist must follow certain standards of care in order to compensate for, correct or modify the structure or the function of the foot when weight-bearing when standing or walking. One of the most common objectives is to limit the mobility of the joint responsible for the pain.
