Introduction |
Le DRESS (« Drug Reaction with Eosinophilia and Systemic Symptoms ») est un syndrome rare déclenché par une réaction immunologique à certains médicaments, qui peut mettre en jeu le pronostic vital par la survenue de manifestations viscérales sévères. Il est caractérisé par un long délai d’apparition entre la prise médicamenteuse et le début des manifestations. Nous rapportons l’observation d’une patiente âgée de 42ans qui a développé un DRESS un mois après le début d’un traitement par allopurinol.
Observation |
Une femme de 42ans était hospitalisée pour un exanthème fébrile avec un œdème du visage, une polyadénopathie, un syndrome mononucléosique, une grande hyperéosinophilie et une cytolyse hépatique. Le diagnostic de DRESS était posé, avec un score RegiSCAR à 5. Le médicament imputable était l’allopurinol, introduit un mois plus tôt. La sérologie HHV-6 était positive en IgM. Deux jours après le début d’une corticothérapie générale, la patiente développait une thrombose de la veine jugulaire interne. En dehors de l’hyperéosinophilie majeure, aucune autre cause favorisant les thromboses n’était détectée. L’évolution était favorable sous anticoagulation efficace et corticothérapie.
Discussion |
De rares cas de thrombose veineuse ont été observés lors de DRESS. L’hyperéosinophilie pourrait être impliquée dans leur survenue. Une anticoagulation préventive systématique peut être nécessaire lors d’un DRESS avec hyperéosinophilie majeure.

Background |
DRESS (Drug Reaction with Eosinophilia and Systemic Symptoms) is a rare syndrome triggered by an immunological reaction to certain drugs and which may be life-threatening as a result of the onset of severe organ involvement. It is characterised by a long period from the time of drug therapy to the onset of actual signs. Herein, we report the case of 42-year-old female patient who developed DRESS one month after beginning allopurinol treatment.
Patients and methods |
A 42-year-old woman was hospitalised for febrile exanthema with facial oedema, polyadenopathy, mononucleosis syndrome, major hypereosinophilia and hepatic cytolysis. A diagnosis was made of DRESS with a RegiSCAR score of 5. The implicated drug was allopurinol, which had been initiated one month earlier. HHV-6 IgM serology was positive. Two days after the start of systemic corticosteroids, the patient developed thrombosis of the internal jugular vein. Other than major hypereosinophilia, no other factors favouring thrombosis were detected. A favourable outcome was achieved under effective anticoagulants and corticosteroids.
Discussion |
They have been rare reports of venous thrombosis during DRESS. Hypereosinophilia can be involved in the onset of this condition. Prophylaxis with systemic anticoagulants may be necessary in DRESS involving major hypereosinophilia.
