Résumé |
Introduction |
La contamination par le papillomavirus, responsable d’une importante morbidité, peut être prévenue par une vaccination spécifique (vaccination anti-HPV). Le Haut conseil à la santé publique (HCSP) a publié le 15 janvier 2013 une nouvelle recommandation abaissant l’âge de vaccination de 16ans à la tranche d’âge 11–14ans, permettant ainsi la co-vaccination anti-HPV et vaccin tétravalent ou trivalent diphtérie tétanos poliomyélite (DTP) ou diphtérie tétanos coqueluche poliomyélite (DTCP). L’objectif de notre étude était d’analyser si la publication de cet avis avait permis de modifier cette co-vaccination.
Méthode |
Une étude descriptive transversale à partir d’une extraction de la base « système national d’information inter-régimes de l’Assurance maladie » SNIIRAM à l’échelon de la région Midi-Pyrénées a été menée pour décrire la variation de co-délivrance des vaccins anti-HPV et vaccins trivalents ou tétravalents DTP ou DTCP sur deux périodes de 1 an autour de la date de publication de cet avis.
Résultats |
Sur la période étudiée 35 647 doses vaccinales ont été remboursées soit 18 230 schémas vaccinaux anti-HPV débutés. Au total, 3287 co-délivrances avec un vaccin trivalent ou tétravalent DTP ou DTCP ont été remboursées : 1406 (16,4 %) avant la publication du HCSP et 1881 (19,4 %) après (p<0,01).
Discussion |
L’avis du HCSP a eu un impact rapide sur l’augmentation de co-délivrance vaccin anti-HPV et vaccin trivalent ou tétravalent DTP ou DTCP. La co-vaccination des jeunes filles n’a pas été un frein à la couverture vaccinale anti-HPV.

Introduction |
Antipapillomavirus vaccination is used to prevent genital infection by papilloma virus, responsible for a high morbidity. In France, the High Council for Public Health published on September 28th 2012 a new guideline decreasing the age of vaccination from 16 to between 11 to 14 allowing a joint vaccination with mandatory tetravalent or trivalent (against diphtheria, tetanus, pertussis and poliomyelitis Tdap-IPV or Td/IPV booster) vaccination. Our study aimed to assess if this guideline changed the rate of joint vaccinations.
Methods |
A descriptive before/after study was conducted on French health insurance reimbursement database at the regional level (Midi-Pyrénées area). The study period comprised 1 year before and 1 year after the publication of the new guideline. We assess the rate of co-vaccinations in these 2 periods and compared them by a Chi2 test.
Results |
On the study period, 35,647 vaccines were reimbursed corresponding to 18,230 complete vaccinations. Concerning co-vaccinations, 3287 were reimbursed: 1406 (16.4%) before the publication of the guideline and 1881 (19.4%) after (P<0.01).
Discussion |
The new guideline was accompanied by a rapid increase in the rate of co-vaccinations. It did not diminish the rate of vaccination by antipapillomavirus.
