COLHEMI: An experiment of intensive reeducation for hemiplegic children associating constraint-induced therapy, bimanual training and goal-oriented therapy

En s’inspirant des expériences internationales, nous avons proposé en France, pour des enfants hémiplégiques, un modèle de prise en charge intensive mis en œuvre sous forme de « colonie » de rééducation de 2 semaines, qui associait la thérapie induite par la contrainte et l’entraînement bi-manuel intensif, mais incluait aussi un temps de thérapie orientée par les buts : COLHEMI. Ce programme a été évalué à J0, J15 (à l’issue immédiate du programme) et à 6 mois, avec des outils d’évaluation des capacités globales (EMFG, IDE), des capacités manuelles (Force de préhension par E-link, Box and Blocks test, test de Melbourne), de la performance bi-manuelle (AHA, CHEQ) et de la satisfaction. Six enfants ont été inclus dans la première session de COLHEMI (2 garçons, 4 filles, âge moyen 7,5 ans - min 6 ans et max 9 ans, 6 hémiplégies droites). Les résultats préliminaires montrent des bénéfices fonctionnels variables mais réels pour chacun des 6 enfants, et une évolution favorable du CHEQ. Les enfants et les parents ont rapporté une grande satisfaction quant à la qualité du programme COLHEMI.

Being inspired by international experiments, we proposed in France, for hemiplegic children, a model of intensive care implemented in a 2 weeks-rehabilitation camp, which associated constraint-induced therapy and bimanual intensive training, but also included a time of goal-oriented therapy: COLHEMI. This program was estimated at J0, J15 (immediate exit of the program) and at 6 months, with tools assessing global capacities (EMFG, IDE), manual capacities (grip strength by E-link, Box and blocks test, Melbourne test), bimanual performance (AHA, CHEQ) and satisfaction. Six children were included in the first session of COLHEMI (2 boys, 4 girls, average age 7.5 years–min. 6 years and max. 9 years old, 6 right hemiplegias). The preliminary results show variable but real functional profits for each of 6 children, and a favorable evolution of the CHEQ. The children and the parents reported a big satisfaction regarding the quality of the program COLHEMI.
