Le syndrome de Weill Marchesani est une affection congénitale qui associe une microsphérophakie et des anomalies squelettiques. Les auteurs rapportent le cas d'un patient de 19 ans, issu d'un mariage consanguin, qui a consulté pour une baisse progressive de l'acuité visuelle. Au plan général, il présentait un aspect trapu avec retard statural, hypertrophie musculaire, brièveté des mains et des pieds et rigidité articulaire. L'examen ophtalmologique a montré une acuité visuelle limitée à la perception des mouvements de la main à droite non améliorable par la correction du vice de réfraction, et à gauche une absence de perception lumineuse. La pression intraoculaire était de 36 mmHg à droite et 40 mmHg à gauche. La chambre antérieure était irrégulière avec irido-phakodonesis et microsphérophakie des deux cristallins. La zonule était partiellement rompue à droite. L'angle iridocornéen était étroit. Le fond d'oeil a montré à droite une pâleur papillaire avec excavation de 9/10 et à gauche une papille totalement excavée. Le bilan cardio-vasculaire a noté une maladie aortique d'origine rhumatismale. Sur le plan thérapeutique, une intervention combinée (phacophagie avec vitrectomie antérieure et trabéculectomie) a été effectuée à droite. Le syndrome de Weill Marchesani est une affection congénitale rare dont la transmission serait autosomique récessive. Le pronostic visuel est dominé par le glaucome secondaire dû au blocage pupillaire par le cristallin mobile. L'observation rapportée illustre la gravité de l'évolution spontanée du syndrome de Weill Marchesani justifiant l'extraction du cristallin avant la survenue des complications.
Weill Marchesani syndrome: a case report |
Weill Marchesani syndrome is a congenital disease that combines microspherophakia and skeletal abnormalities. The authors report a 19-year-old male, born of a consanguineous marriage, with a progressive decrease in visual acuity. The general examination showed a squat look, dwarfism, muscle hypertrophy, short hands and feet, and joint stiffness. The ophthalmological examination showed that visual acuity was limited to hand motion in the right eye despite correction and no perception of light in the left eye. Intraocular pressure was 36mmHg in the right eye and 40mmHg in the left eye. The anterior chamber was irregular with iridophakodonesis and microspherophakia of both lenses. The zonula was partially ruptured in the right eye. The iridocorneal angle was narrow. Fundus eye examination showed a pale optic disc with an excavation of 9/10 on the right. In the left eye, the optic disc was totally excavated. Cardiovascular check-up revealed rheumatic aortic valvular cardiopathy. The therapy consisted of combined surgery: phakophagia with anterior vitrectomy plus trabeculectomy operated on the right eye. Weill Marchesani syndrome is a rare congenital affection with a recessive autosomal transmission. The visual prognosis is dominated by secondary glaucoma due to pupillary blockage by the mobile eye lense. This observation illustrates the seriousness of spontaneous progression in Weill Marchesani syndrome, justifying the necessity of lens extraction before the onset of complications.
