Les auteurs décrivent un cas de cysticercose oculaire intravitréenne unilatérale découvert dans le service d'ophtalmologie de l'hôpital Edouard Herriot à Lyon et dont le diagnostic s'est avéré particulièrement difficile.
La symptomatologie débute par une uvéite postérieure avec hyalite qui s'est rapidement aggravée en 10 jours par une panuvéite malgré les traitements antibiotiques, corticoïdes puis antiviraux successivement institués. L'interrogatoire très pauvre ne donne que peu d'élément d'orientation. Les bilans sérologiques et radiologiques répétés se sont toujours avérés négatifs. Le diagnostic n'a pu être posé qu'après vitrectomie, ce qui a permis de mettre en évidence le kyste cysticerquien. Cette découverte a été d'autant plus étonnante que le patient est d'origine montagnarde française du patient et qu'il a omis de signaler un voyage aux Antilles 3 mois auparavant. Une revue de la littérature a permis de mettre en route rapidement un bilan parasitaire postopératoire à la recherche de localisations cérébrales, musculaires, cardiaques et sous-cutanées et d'instituer un traitement par Praziquantel (50 mg/kg/j) sous corticothérapie (1 mg/kg/j) afin de traiter les éventuels autres cysticerques. L'oeil a évolué vers une phtisie très inflammatoire ce qui a conduit à une éviscération.

The authors describe a case of intraocular cysticercosis discovered in the ophtalmology department ef Edouard Herriot Hospital. With no biological signs, the diagnosis was made after vitrectomy because the visit was late in the course of the disorder, the patient lived in the mountains, and he had failed to mention a trip to the West Indies 3 months before.
The symptomatology began with uveitis worsened in 10 days by greater inflammation in spite of successive corticosteroids, antibiotic, and antiviral therapy. Repeated examination and serological and radiological investigations did not help establish the etiology. Only vitrectomy provided the diagnosis, the vitreous containing cysticercus. This discover was all the more surprising that the patient lived in the French Alps and a trip 3 months before had been forgotten. A review ol the literature allowed us to rapidly start a postoperative parasite examination to check for cerebral, muscular, cardiac, and subcutaneous localizations and to start treatment with praziquantel (50 mg/kg per day) with corticotherapy (1 mg/kg per day). The eye evolved toward a very inflammatory phthisis, which led to evisceration.
