Abstract |
Snapping triceps corresponds to subluxation of the medial head of the triceps brachii over the medial epicondyle during active elbow flexion. It is a rare and misleading condition that is often associated with ulnar nerve instability. The latter can result in the snapping triceps being missed. Diagnosis requires a detailed clinical examination. Surgical treatment consists of anterior translocation of the nerve with an adipose flap to stabilize it and transfer of the distal attachment of the medial head of triceps brachii.

Le “snapping triceps” est la luxation du chef médial du triceps brachii au-dessus de l’épicondyle médial. Peu fréquent, le diagnostic est souvent méconnu. S’associant souvent à une instabilité du nerf ulnaire, cette entité peut faire ignorer le diagnostic de “snapping triceps”, aboutissant à un traitement incomplet. Toute instabilité du nerf ulnaire doit faire rechercher un “snapping triceps”. Le diagnostic repose sur un examen dynamique par contraction musculaire contre résistance. Le traitement consiste en une résection partielle ou un transfert de l’insertion du chef médial du triceps brachii, associé au besoin à une transposition du nerf ulnaire stabilisée par un lambeau graisseux.
