Observance of pre-IVG contraceptive methods and analysis of post-IVG behavioral changes

Contexte |
Depuis plusieurs années, en France, le nombre d’interruptions volontaires de grossesse (IVG) est stable, malgré l’usage largement répandu de divers moyens de contraception, et la maîtrise de la reproduction qu’ils permettent aux femmes.
Objectifs |
Nous avons effectué un état des lieux des comportements contraceptifs et de l’observance des méthodes avant puis après une IVG, et nous avons notamment analysé les défauts d’usage et la prévalence des méthodes utilisées.
Méthodes |
Il s’agit d’une étude observationnelle, descriptive et analytique, réalisée entre le 1er octobre 2016 et le 28 février 2017 à la maternité régionale universitaire de Nancy. Les 70 patientes inclues dans notre cohorte avaient réalisé une IVG, médicamenteuse ou chirurgicale, ou effectué leur consultation de contrôle post-IVG à la maternité, au cours de cette période. Le recueil des données s’est effectué par le biais de questionnaires qui avaient été distribués aux patientes, et dont l’analyse reposait sur de multiples variances.
Résultats |
En pré-IVG, la pilule était majoritairement utilisée, suivie par l’emploi du préservatif masculin. Près d’un quart des patientes n’utilisait pas de moyen de contraception et l’usage de dispositifs intra-utérin (DIU) et d’implants était mineur. La quasi-intégralité de notre cohorte avait commis des erreurs contraceptives durant les 3 derniers mois pré-IVG, et une part importante de notre population avait changé ou interrompu sa contraception au cours des 6 derniers mois. En post-IVG, l’usage de DIU ou d’implants augmentait nettement avec la moitié de notre cohorte qui les employait, suivi de près par l’utilisation de la pilule. L’absence de contraception ne concernait plus que 4 patientes, et l’usage du préservatif reculait considérablement.
Conclusion |
Les erreurs contraceptives se révèlent nombreuses avant l’avortement et les comportements contraceptifs sont différents, selon qu’ils étaient analysés en pré ou post-IVG. Les prescriptions post-IVG favorisent les méthodes réversibles de longue durée d’action et semblent davantage adaptées au mode de vie et à la sexualité des femmes ou du couple.

Context |
For several years in France, the number of voluntary termination of pregnancy is stable despite the use of many different contraceptive methods that allow patients to control the fertility.
Objectives |
We did a check-up of contraceptives behaviours and methods observance before and after voluntary termination of pregnancy. We also analysed non-use and prevalence of the used methods.
Methods |
From October 1st 2016 to February 28th 2017, we did an observational, descriptive and analytic study at the Regional Maternity hospital of Nancy. A total of 70 patients were interviewed. During this period, the patients received a voluntary termination of pregnancy, with drugs or surgical operation, or realised a check-up consultation after having received a voluntary termination of pregnancy. We collected data from questionnaires distributed to patients. The analysis was based on multiple variables.
Results |
Before the voluntary termination of pregnancy, patients used mostly pills and male condom. Almost a quarter of patients didn’t use any contraceptive methods. The use of intrauterine devices and implants was low. Almost all the patients make contraceptives mistakes during the 3 last months before their voluntary termination of pregnancy. Many of them changed or stopped their contraception during the last 6months. After the voluntary termination of pregnancy, the use of intrauterine devices and implants surged: the half of our population chose them, closely followed by the pill. The lack of contraception concerned only 4 patients and the use of condom significantly felt.
Conclusion |
There are many several contraceptives mistakes before the voluntary termination of pregnancy. Contraceptive behaviours are different before or after the voluntary termination of pregnancy. Prescriptions after the voluntary termination of pregnancy stimulate reversible methods of long-term actions and seem to be more adapted to the way of life and the sexuality of women or couple.
