Résumé |
Objectif du travail |
Déterminer la valeur ajoutée d’une tomoscintigraphie du tronc couplée au scanner dans le diagnostic des métastases osseuses (MO), comparer ses résultats à ceux du balayage du corps entier (BCE) et préciser l’impact diagnostique tout en prenant en considération le surcoût de l’irradiation additionnelle imputable au scanner.
Patients et méthodes |
Étude prospective incluant 150 patients suivis pour une pathologie néoplasique entre juin 2013 et décembre 2014. Tous les patients ont bénéficié d’un BCE suivi par une TEMP/TDM du tronc.
Résultats |
Au total, 1375 lésions ont été notées dont 15,7 % n’ont pas été vues au BCE. Le taux des lésions indéterminées est passé de 18,7 % au BCE à 1,9 % à la TEMP/TDM. Les taux de concordance et de discordance entre le BCE et la TEMP/TDM dans la caractérisation des lésions selon leur nature étaient, respectivement, de 63,9 % et 1,5 %. Le gril costal et le rachis dorsal représentaient les premières localisations des lésions suspectes de malignité en TEMP/TDM. L’analyse par patients a montré une réduction du nombre des examens classés indéterminés au BCE de 69 %. Elle a permis également de mieux préciser l’extension métastatique sans modification du statut chez 32 patients et de changer le statut du patient dans 6 cas. La TEMP/TDM n’a pas apporté d’information supplémentaire au BCE chez 47 patients. La dose efficace de 4mSv était imputable à l’imagerie d’émission alors que le scanner a délivré une dose efficace moyenne estimée à 10,4mSv.
Conclusion |
La TEMP/TDM du tronc est d’un apport indiscutable puisqu’elle améliore la qualité du compte rendu, réduit le nombre d’examens indécis et augmente le niveau de confiance au profit d’une irradiation non considérable.

Aim of the study |
Show the added value of SPECT-CT of the trunk in the diagnosis of bone metastases, compare its results to those of whole body scintigraphy (WBS) and specify the diagnostic impact while taking into account the cost of additional irradiation attributable to the scanner.
Patients and methods |
Prospective study including 150 patients presenting neoplasic pathology between June 2013 and December 2014. All patients have had WBS followed by a SPECT-CT of the axial skeleton.
Results |
A total of 1375 lesions were noted, of which 15.7 % were not seen in the WBS. The rate of indeterminate lesions increased from 18.7 % in the WBS to 1.9 % in the SPECT-CT. The concordance and discordance rates between WBS and SPECT-CT in the characterization of lesions according to their nature were 63.9 % and 1.5 %, respectively. The rib cage and the thoracic spine were the first localizations of the suspicious lesions of malignancy in SPECT-CT. Patient analysis showed a reduction in the number of scintigraphies classified as indeterminate to the WBS of 69 %. It also made it possible to better specify the metastatic extension without modification of the status in 32 patients and to change the status of the patient in 6 cases. SPECT-CT did not provide additional information to the WBS in 47 patients. The effective dose of 4mSv was due to emission imaging while the scanner delivered an average effective dose estimated at 10.4mSv.
Conclusion |
The SPECT-CT of the trunk has an indisputable contribution since it improves the quality of the report, reduces the number of undecided exams and increases the confidence level in favor of a non-significant irradiation.
