Les inhibiteurs de checkpoints immunitaires anti-PD-1, anti-PD-L1 et anti-CTLA-4 ont connu un développement important dans de multiples indications et ont révolutionné la prise en charge de certains patients atteints d’un cancer. L’immunothérapie des cancers a ainsi occupé une place centrale dans les présentations du congrès de l’ASCO 2017. L’identification des patients qui pourrait bénéficier le plus des inhibiteurs de checkpoints immunitaires reste un enjeu clé. La valeur prédictive du statut PD-L1 reste insuffisante pour sélectionner les patients qui pourraient répondre à l’immunothérapie. Une attention particulière est portée sur la recherche de nouveaux biomarqueurs prédictifs de réponse (INF-γ, charge mutationnelle) dans l’objectif de mieux sélectionner les répondeurs. Les inhibiteurs de checkpoints immunitaires ont surtout été développés en monothérapie. Cependant, le faible taux de réponse, compris entre 10 et 30 %, et l’émergence de résistance contribuent à l’incrémentation de nouvelles stratégies de combinaisons thérapeutiques. Cet article propose une synthèse des résultats d’essais de combinaison de deux immunothérapies, et d’association d’une immunothérapie à la chimiothérapie conventionnelle, à la radiothérapie ou aux thérapies moléculaires ciblées actives sur la voie d’addiction oncogénique.

Immune checkpoint inhibitors anti-PD-1, anti-PD-L1 and anti-CTLA-4 have been in development in several indications and have changed the face of cancer patients’ management. Cancer immunotherapy was central in ASCO's meeting 2017. The identification of patients who could benefit most from immune checkpoint inhibitors is essential. The predictive value of PD-L1 status remains insufficient to select patients who could respond to immunotherapy. An extended search for new biomarkers predictive of response (INF-γ, mutational load) is ongoing, in order to better select responders. Immune checkpoint inhibitors have mainly been developed as monotherapy. However, the low response rate, between 10 and 30%, and the occurrence of resistance, contributes to the increment of new therapeutic strategies. This review summarizes the results of combination trials of two immune checkpoint inhibitors, combination of immunotherapy with conventional chemotherapy, radiotherapy or targeted therapies active on the oncogenic addiction pathway.
