Osteoarticular manifestations after sting, scratch, bite, and due to rare and emergent agents

Les manifestations ostéo-articulaires secondaires à des piqûres, morsures, griffures sont relativement rares. Il ne faut pas les méconnaître car un traitement antibiotique adapté, voire un traitement chirurgical, est nécessaire. Un interrogatoire policier recherche une blessure et une contamination tellurique (Pantoea agglomerans [P. agglomerans] et nocardioses), par morsure de chien (Capnocytophaga canimorsus [C. canimorsus], Pasteurella multocida [P. multocida]), par griffure de chat (Bartonella henselae [B. henselae], P. multocida), avec des carcasses d’oiseaux, d’ovins, de porcins (Erysipelothrix rhusopathiae [E. rhusopathiae]), un contact avec de l’eau contaminée par les urines de rongeur (Streptobacillus moniliformis [S. moniliformis] Spirillum minus [S. minus], Leptospira interrogans [L. interrogans]). L’identification du germe est parfois difficile, mais est essentielle pour adapter le traitement antibiotique. Une piqûre peut également provoquer une arthrite en l’absence de germe, créant de l’inflammation au sein même de l’articulation. Un corps étranger, souvent radio transparent, peut être vu en échographie. Une arthroscopie pour évacuer le corps étranger et effectuer une synovectomie est souvent nécessaire. Les infections à mycobactéries atypiques surviennent plus volontiers chez des patients immunodéprimés, mais pas uniquement. Une blessure avec du matériel de pêche ou contaminée par de l’eau oriente vers Mycobacterium marinum (M. marinum) et un antécédent d’infiltration vers Mycobacterium kansasii (M. kansasii). Qu’il s’agisse d’une arthrite ou d’une ténosynovite, une chirurgie est souvent nécessaire en plus d’un traitement antibiotique adapté. Les prélèvements faits en cours de chirurgie aident au diagnostic. Les atteintes ostéoarticulaires après morsure de tique sont assez rares en France en dehors de la maladie de Lyme. En fonction des symptômes une babésiose ou une rickettsiose doivent être recherchées. Parmi les affections rhumatologiques secondaires aux piqûres d’arthropodes, le chikungunya offre un modèle de virus arthritogène, déclenchant ou réactivant d’authentiques rhumatismes inflammatoires.

Osteoarticular manifestations after a sting, a bite or a scratch are rare but the diagnosis should not be neglected. Targeted antibiotic therapy is mandatory. Surgery might help the diagnosis with adequate biopsy and is sometimes essential for recovery. A detective work is needed to identify a history of wound. Pantoea agglomerans (P. agglomerans) and nocardiosis are seen after a telluric wound, Capnocytophaga canimorsus (C. canimorsus) and Pasteurella multocida (P. multocida) after a dog bite, Bartonella henselae (B. henselae) and P. multocida after a cat scratch, Erysipelothrix rhusopathiae (E. rhusopathiae) when the wound is caused by a pig bone or a fishbone. A contact with water soiled with rat urine during work or leisure activity could indicate infection with Streptobacillus moniliformis (S. moniliformis) Spirillum minus (S. minus), Leptospira interrogans (L. interrogans). Bacteriological documentation might be difficult but is often mandatory to ascertain diagnosis and to adapt antibiotics. A sting can be responsible for a non-infectious arthritis. Inflammation is directly mediated by foreign body and is called “thorn arthritis”. Foreign body is often not seen on conventional radiography, hardly seen in ultrasonography, and surgery is needed to extract it and sometimes to perform synovectomy. Atypical mycobacteria are seen in patients with immunodepression but not only. Contact with fish or fishing material brings Mycobacterium marinum (M. marinum) to mind and a history of joint injection evoke Mycobacterium kansasii (M. kansasii). Surgery is frequently needed for diagnosis and treatment of tenosynovitis or arthritis caused by atypical mycobacteria. Tick-borne arthritis are mainly caused by Borrelia burgdorferi but can also involve Babesia divergens or Rickettsia conorii. Among arthropod-borne viruses, chikungunya provides a model of infection-triggered inflammatory joint disease.
