Matériels et méthodes |
En mars 2017, une enquête « un jour donné » est réalisée au sein d’un centre hospitalier général de 828 lits. Il s’agit d’étudier les modalités de prescription des statines chez tous les patients hospitalisés et traités par un médicament de cette classe thérapeutique. Les paramètres recherchés sont : âge et sexe du patient, unité d’hospitalisation, concordance entre les statines prescrites à domicile et à l’hôpital, bon choix de la statine en fonction de l’indication, adaptation de la posologie au bilan lipidique et au risque cardiovasculaire, antécédent iatrogénique sous statine, statine prescrite en sortie d’hospitalisation.
Résultats |
Quatre-vingt-onze prescriptions de statines sont recensées et analysées. L’objectif de la concordance entre la substance active prescrite et l’indication est atteint dans moins de la moitié des cas.
Discussion |
Parmi les objectifs secondaires, la bonne poursuite du traitement de ville pendant le séjour et l’absence d’iatrogénie chez les patients concernés sont des points positifs. Par contre, la posologie de statine est la plupart du temps mal corrélée au bilan lipidique. Enfin, en cas de substitution hospitalière dans le seul but de se conformer au livret du médicament, le traitement de sortie reprend plus souvent le traitement prescrit pendant l’hospitalisation que le traitement initié par le médecin de ville.

Materials and methods |
In March 2017, a “one day” survey was conducted in an 828-bed general hospital. The aim was to study the prescription of statins in all hospitalized patients treated with a drug of this therapeutic class. Study parameters comprised: age and gender, hospital unit, concordance between statins prescribed at home and in hospital, correct choice of statin according to indication, dose adjustment to lipid profile and cardiovascular risk, adverse drug reactions associated with statins, and statins prescribed at discharge.
Results |
Ninety one statin prescriptions were identified and analyzed. The main endpoint of concordance between prescribed molecule and indication was found in fewer than half of cases.
Discussion |
Among secondary endpoints, continuation of treatment at admission during the hospital stay and absence of adverse drug reactions were positive points. In contrast, statin dosage was poorly correlated with lipid profile. Lastly, in case of in-hospital drug switch for the sole purpose of complying with the drug formulary, the discharge treatment was more often the statin prescribed in hospital than the drug initiated in the community.
