• | Results of phase III clinical trials assessing maribavir, brincidofovir, and letermovir in the prophylactic treatment of CMV infections following hematopoietic stem cell transplantations were disappointing for maribavir and brincidofovir, but letermovir showed efficacy and good clinical tolerability. Letermovir will thus soon be commercialized. Maribavir is still being evaluated at the highest possible dose in the preemptive/curative treatment of CMV infections following transplantation in adolescents and adults. |
Abstract |
Cytomegalovirus (CMV) infection is a common complication in immunocompromised patients, especially after hematopoietic stem cell or solid organ transplantation. Therapeutic antiviral options [(val)ganciclovir, foscarnet, cidofovir] are still limited and can expose to severe toxicities. Moreover, prolonged antiviral drug exposure and ongoing viral replication are key factors in the development of antiviral drug resistance. After many years of few tangible advances in terms of new antiviral drugs, we are now experiencing an exciting period characterized by a series of phase III clinical trials incorporating three novel agents: maribavir, brincidofovir, and letermovir. This article summarizes the current state of the prevention and treatment of CMV infections as well as data of investigational drugs in clinical development.

L’infection à cytomégalovirus (CMV) est une cause majeure de morbidité et de mortalité chez les patients immunodéprimés, notamment après greffe de cellules souches hématopoïétiques ou d’organe solide. Les traitements antiviraux actuellement disponibles ([val]ganciclovir, foscarnet, cidofovir) sont en nombre très limité, exposent à un risque de toxicité non négligeable et de sélection de virus CMV résistants en cas de réplication virale prolongée sous traitement. Des progrès récents ont été faits dans le développement de trois nouvelles molécules antivirales (maribavir, brincidofovir, létermovir) qui font actuellement (ou ont récemment fait) l’objet d’essais cliniques de phase III et pourraient être particulièrement intéressantes du fait de leur profil de tolérance et de leur activité préservée vis-à-vis de certains CMV résistants aux molécules actuelles. Nous présentons ici, en miroir des données concernant les molécules antivirales actuellement disponibles, celles relatives à ces trois nouveaux antiviraux qui pourraient trouver une place parmi les perspectives thérapeutiques futures du traitement des infections à CMV.
