Caregiver-patient communication and evolution of chronic diseases at the University Clinics of Kinshasa, DRC

Contexte |
Les patients atteints des maladies dégénératives ont besoin d’être informés de leur situation d’une manière précise en vue de participer dans la prise de décision le concernant, gage d’un résultat escompté. Cette étude avait pour objectif, de vérifier le lien entre la communication soignant–soigné et l’évolution des patients atteints des maladies chroniques à Kinshasa.
Méthode |
Une étude transversale était menée auprès des patients chroniques hospitalisés aux cliniques universitaires de Kinshasa. L’échelle CMM13 a été utilisée pour apprécier la qualité de communication soignant–soigné. L’auto-appréciation de ces patients a permis d’évaluer leur évolution. Les données étaient traitées à l’aide du logiciel Jamovi 2.3.21. Le test exact de Fisher et U de Mann-Whitney (α=0,05) ont permis la vérification de lien entre les variables.
Résultats |
L’âge moyen de participants était de 59,9±16,5ans, sex-ratio de 1,2 femme pour 1 homme. Le diabète sucré (47,9 %) et l’hypertension artérielle (44,5 %) étaient les principaux motifs d’hospitalisation. De l’évolution, 73 % ont témoigné l’amélioration de leur état de santé depuis le début de leur hospitalisation. Le score médian de la communication de qualité était de 23,1 (IQR=57,7) chez ces patients contre 15,4 (IQR=23,1) chez ceux ayant signalés l’absence d’amélioration (p=0,050). Cette évolution auto-appréciée était significativement liée au fait que le patient soit encouragé par le soignant à s’exprimer (p=0,017), à l’attention du personnel pendant les échanges (p=0,001), l’empathie (0,003) et la clarté (p=0,013).
Conclusion |
Cette étude montre que l’évolution de l’état de santé de patients souffrants de maladies chroniques hospitalisés aux cliniques universitaires de Kinshasa est fonction de la qualité de communication dont ils sont bénéficiaires. Ainsi, le soignant devrait être disponible et attentif pendant les échanges avec le patient.

Patients with degenerative diseases need to be informed of their situation in a precise way in order to participate in decision-making concerning them, guaranteeing an expected result. The main objective of this study was to verify the link between caregiver-patient communication and the evolution of patients with chronic diseases in Kinshasa.
Method |
A cross-sectional study was conducted among chronic patients hospitalized at the University Clinics of Kinshasa. The CMM13 scale was used to assess the quality of caregiver-patient communication. The self-assessment of these patients made it possible to evaluate their evolution. Data were processed using Jamovi 2.3.21 software. Fisher's exact test and Mann-Whitney U (α=0.05) allowed verification of the link between variables.
Results |
The median age of participants was 59.9±16.5 years, sex ratio of 1.2 women for 1 man. Diabetes mellitus (47.9 %) and arterial hypertension (44.5 %) were the main reasons for hospitalization. About evolution, 73 % testified to the improvement of their health since the beginning of their hospitalization. The median quality communication score was 23.1 (IQR=57.7) in these patients versus 15.4 (IQR=23.1) in those reporting no improvement (P=0.050). This self-perceived evolution was significantly related to the fact that the patient is encouraged by the caregiver to express himself (P=0.017), to the attention of the staff during the exchanges (P=0.001), empathy (0.003) and clarity (P=0.013).
Conclusion |
This study shows that the evolution of patients suffering from chronic diseases hospitalized at the University Clinics of Kinshasa is function of the quality of communication from which they benefit. Thus, the caregiver should be available and attentive during discussions with the patient.
