Résumé |
L'optimisation des fonctions intestinales au cours de l'insuffisance intestinale est une approche alternative à la nutrition parentérale prolongée. La trophicité intestinale est stimulée par l'apport alimentaire lui-même, les polyamines, et certains pharmaco-nutriments azotés (glutamine, arginine, oxoglutarate d'ornithine), mais aussi par des facteurs de croissance hormonaux ou locaux. L'hormone de croissance et le glucagon-like peptide-2 (GLP-2), mais également l'insulin growth factor, l'epidermal growth factor et le transforming growth factor-alpha stimulent la prolifération cellulaire intestinale, le transforming growth factor-beta l'inhibe. Des traitements associant glutamine, hormone de croissance et modifications diététiques ont été évalués chez des patients présentant un grêle court, avec des résultats discordants. L'utilisation du GLP-2, seul ou en association, représente une perspective séduisante qui reste à évaluer. Le développement rapide des connaissances dans le domaine des facteurs de croissance intestinaux et des pharmaconutriments devrait permettre de déboucher sur une stratégie renouvelée de prise en charge de l'insuffisance intestinale.

During intestinal insufficiency, improving intestinal functions may limit the needs for prolonged parenteral nutrition. Gut trophicity is supported by the intake of nutrients, polyamines, and nitrogenous pharmaconutrients such as glutamine, arginine and ornithine oxoglurate, but also growth factors acting as hormones or locally. Intestinal cell proliferation is stimulated by growth hormone and glucagonlike peptide-2, as well as by the insulin growth factor, epidermal growth factor, transforming growth factor-alpha, while it is inhibited by transforming growth factor-beta. The evaluation of combined therapies with glutamine, growth hormone and a specialized diet in short bowel patients has yielded conflicting results. The clinical use of GLP-2, alone or in combination, looks promising but need to be evaluated. The knowledge about intestinal growth factors and pharmaconutrients is increasing rapidly and should lead to innovative therapeutic strategies of intestinal insufficiency.
