Incident anecdotique lors de l’utilisation d’un aérosol de bronchodilatateur - 13/12/10

Résumé |
L’asthmatique a très souvent recours aux aérosols à tel point que de nombreuses précautions d’usages sont négligées telles que refermer l’embout après usage. Nous rapportons l’observation d’une patiente de 22ans qui à la suite de l’inhalation d’une bouffée de bronchodilatateur pour calmer une crise d’asthme, a présenté une dyspnée d’aggravation brutale qui a cédé après des quintes de toux. L’interrogatoire révèle la notion d’inhalation accidentelle d’une pince à cheveux qui était enclavée dans le conduit d’aération de l’aérosol. L’extraction du corps étranger a été réalisée difficilement par une bronchoscopie rigide. Ce cas illustre la nécessité de toujours reboucher l’embout de son spray après utilisation et de vérifier le contenu de son conduit avant administration. Des gestes simples qui permettent d’éviter ce type d’incident aux complications parfois dramatiques nécessitant dans certains cas le recours à la chirurgie.

Asthmatics use aerosols devices so often that some of the precautions to be taken before usage are disregarded, for example, they forget to close the nozzle after using the device. We report the case of a 22-year-old patient who, following inhalation of a dose of bronchodilator to control an asthma attack, presented with sudden worsening of the asthma, which then stopped after a bout of coughing. Questioning revealed that the patient had accidentally inhaled a hair clipper, which had entered into the airway of the aerosol device. Extraction of this foreign body was accomplished, with difficulty, using a rigid bronchoscope. This case illustrates the need to always close the opening of the nozzle of the aerosol device after using it and to be sure that the duct is clear before administration of the aerosol. Simple measures can prevent this type of incident, which can sometimes lead to dramatic complications, which may require surgical intervention in some cases.
