Intérêt de la thyroïdectomie dans la prise en charge des kystes du tractus thyréoglosse dégénérés - 01/01/04

Objectifs. - Discuter l'intérêt de la thyroïdectomie dans la prise en charge des cancers papillaires découverts au sein d'un kyste du tractus thyréoglosse. (KTT).
Patients et méthodes. - Étude rétrospective portant sur l'ensemble des patients opérés d'un KTT entre 1985 et 2002.
Résultats. - quatre patients présentaient un carcinome papillaire du KTT. Deux patients ont bénéficié d'une thyroïdectomie dans le mois suivant la chirurgie du kyste, pour l'un d'entre eux il existait un cancer papillaire de la thyroïde. Le troisième patient a développé un cancer papillaire de la thyroïde quinze ans après l'exérèse du kyste dégénéré. Le dernier patient a bénéficié d'un traitement freinateur et ne présente pas de complication thyroïdienne après onze ans de surveillance.
Conclusion. - Les foyers carcinomateux microscopiques sans invasion de la paroi du kyste peuvent bénéficier d'une ablation de type Sistrunk seule associée à un traitement freinateur, sous couvert d'une surveillance à très long terme exposant au risque de perte de suivi. Dans tous les autres cas le traitement que nous prônons comme beaucoup d'autres équipes est la thyroïdectomie suivie d'une radiothérapie interne par iode radioactif.

Purpose of study. - To discuss the autors' experience with thyroglossal duct (TD) carcinoma and expose the interest of the thyroïdectomy in the management of this entity.
Patients and methods. - A retrospective review of all patients with the diagnosis of TD operated on from 1985 to 2002 was performed.
Results. - Four cases of papillary TD carcinoma were identified. Two patients were treated by a Sistrunk procedure associated with total thyroïdectomy. One patient needed a thyroïdectomy fifteen years after the initial management of the papillary thyroglossal duct carcinoma. The last patient had a medical treatment, with no evidence of complication after eleven years of follow up.
Conclusion. - A microscopic focus of papillary carcinoma, without cyst wall invasion, can be managed by a Sistrunk procedure, with the need for long-term follow up. Treatment of all other thyroglossal duct papillary carcinomas should include thyroïdectomy followed by radioactive iodine treatment.
