Objectif de l'étude : L'amaigrissement du patient obèse est un préalable à l'amélioration esthétique de l'obésité ou de ses séquelles par le chirurgien plasticien. Cette étude avait pour but d'évaluer l'efficacité et les avantages du cerclage gastrique par un anneau posé sous coelioscopie.
Patients et méthodes : Trente-neuf patients ayant une obésité massive ont été opérés. Chez les neuf premiers, la restriction gastrique a été obtenue par la technique (ancienne) de Mason. Chez les 30 patients suivants, un anneau de Kuzmak a été posé sous coelioscopie, à quelques centimètres au dessous du cardia. L'étude a porté sur l'évolution du poids et des comorbidités et sur les complications.
Résultats : Tous les patients avaient perdu du poids trois mois après l'intervention (moyenne : - 15 kg). L'amaigrissement s'est poursuivi à six mois (moyenne : - 26 kg), puis à un an (moyenne : - 35 kg). Deux patients ont eu des vomissements postprandiaux qui ont justifié un élargissement du calibre de l'anneau. Un anneau mal toléré a été retiré dix jours après l'intervention.
Conclusion : Cette nouvelle technique chirurgicale est efficace pour obtenir un amaigrissement chez les patients ayant une obésité massive. L'approche coelioscopique de l'estomac réduit les complications pariétales et préserve les possibilités d'une chirurgie plastique ultérieure de l'abdomen.

Morbid obesity: laparoscopic gastroplasty before plastic surgery.
Objectives: Reduction of the weight in obese patients is a pre-condition for the aesthetic improvement of obesity or its after effects by plastic surgery. The aim of this study was to evaluate the effectiveness and advantages of gastric banding, using a ring positioned under laparoscopic guidance.
Patients and methods: Thirty-nine patients with morbid obesity were operated on. For the first nine patients, gastric restriction was obtained by Mason's traditional technique. In the remaining thirty, a Kuzmak ring was positioned a few centimeters under the cardia, under laparoscopic guidance. The study concerned the evolution of weight, associated pathologies, and complications.
Results: Three months after surgery, all patients had lost weight (mean loss : 15 kg). At six months, a mean weight loss of 26 kg was observed. At one year, the mean weight loss was 35 kg. Two patients experienced postprandial vomiting that justified widening of the ring ; one ring was poorly tolerated and was removed 10 days after surgery.
Conclusion: This new surgical technique is effective for reducing the weight of obese patients. The laparoscopic approach to the stomach reduces parietal complications and enables subsequent abdominal plastic surgery to be performed.
