Résumé |
Les changements physiopathologiques du patient agressé sont marqués par une réponse inflammatoire et une réponse metabolique. La phase précoce, est dominée par un stress oxydatif accru, une réponse inflammatoire et des défenses antioxydantes diminuées, et une phase tardive où l'élément central est la stimulation du turn-over protéique et une réponse compensatoire anti-inflammatoire. La stratégie nutritionnelle doit être adaptée à chacune de ces deux phases : supplémentation en antioxydants dans la première et en acides aminés dans la deuxième. Dans les deux cas, la glutamine et la cystéine jouent un rôle majeur. Des travaux ont été menés avec l'utilisation séquentielle de modules nutritionnels prêts à l'emploi, Modulis™ Anti-Ox riche en antioxydants dans la phase précoce et Modulis™ Amino 5 riche en acides aminés dans la phase tardive, avec des résultats encourageants.

Critically ill patients suffer from a changing pathophysiological process that involves an initial inflammatory phase followed by a catabolic response with immunosuppression. During the first phase there is a marked oxidative stress and an inflammatory response that consumes most of the anti-oxidant systems of the organism. During the late phase the major feature is the catabolic status with increase protein turn-over and the compensatory anti-inflammatory response that results in impaired defences for the body. There is a possibility to adapt the nutritional strategies to these changing phases: to provide supplementation of anti-oxidants in the first phase and amino acids during the catabolic phase. In both phases glutamine and cysteine play a central role. Experimental and clinical work have been performed to evaluate the benefits of this changing nutritional strategy. The results of these studies are described and support the use of adapted nutrition with MODULIS AOX and MODULIS AA5.
