Traitement chirurgical des scolioses neuromusculaires - 09/12/11

Le traitement orthopédique des scolioses neuromusculaires a une efficacité et une tolérance souvent limitées, et sert essentiellement comme un moyen d’attente du traitement chirurgical, basé actuellement sur l’arthrodèse vertébrale postérieure instrumentée. L’importance de la déformation rachidienne et sa rigidité peuvent nécessiter, dans certains cas, un abord rachidien antérieur de libération convexe dans un premier temps ou bien une préparation par une traction à travers un halo crânien sur plusieurs semaines, avant la chirurgie d’arthrodèse postérieure. Pour un certain nombre de déformations rachidiennes particulièrement évolutives, se pose la question d’un traitement chirurgical précoce, sous la forme d’une instrumentation sans greffe, dont l’objectif est de redresser la colonne et de la maintenir la plus droite possible, jusqu’à l’âge de l’arthrodèse définitive, au prix d’une intervention annuelle de retention de tige. L’arthrodèse vertébrale ayant pour inconvénients majeurs de stopper la croissance rachidienne et de bloquer définitivement la mobilité du dos, en plus des nombreux risques qu’elle comporte, nous espérons parvenir dans un avenir assez proche, à proposer aux enfants scoliotiques une solution chirurgicale qui permet de traiter la déformation rachidienne, sans pour autant compromettre ni la croissance ni la mobilité de la colonne vertébrale ; tout en épargnant aux patients les risques d’une chirurgie lourde.

The efficiency and tolerance of the conservative treatment of the neuromuscular scoliosis are limited. This treatment is often used to wait for the surgical treatment, based now on the posterior instrumented vertebral arthrodesis. The importance of the spinal deformity and its rigidity can require in some cases a convex anterior spinal release at first or a preparation by traction through a cranial halo over several weeks before the posterior arthrodesis. For some very evolutive cases of spinal deformities an early surgical treatment is necessary such an instrumentation without fusion, the objective of which is to reduce the deformity of the spine and to maintain it the most straight possible up to the age of the definitive arthrodesis by performing an annual rod retention. The vertebral arthrodesis having for major inconveniences to arrest the spinal growth and to definitively destroy back’s mobility of the patient besides the several risks which it induces. That’s why we hope to propose to the scoliotic children in a rather close future a surgical solution which allows to treat the spinal deformity, without compromising either the growth or the mobility of the spine; while saving the patients from the risks of a complicated surgery.
