Le conflit fémoroacétabulaire

La notion de conflit antérieur repose sur un faisceau d’arguments :

• cliniques : douleur inguinale et son exacerbation en flexion, adduction et rotation médiale, diminution de la rotation médiale éventuellement, claquement ou craquement. Douleurs en position assise ;
• sur la radio de face : tête asphérique, coxa vara, rétroversion du cotyle, coxa profunda ;
• sur le profil de Dunn : absence d’offset, méplat à la jonction tête-col, voire tuméfaction ;
• scanner : rétroversion de la partie supérieure du cotyle, faible antéversion du fémur ;
• arthroIRM : modification de signal sur le col du fémur à la jonction tête-col, lésion du bourrelet, modification du signal de l’os sous-chondral sur la partie antérieure du cotyle ou sur la corne postérieure.

Anterior impingement is an entity based on a group of arguments:

• clinical presentation: groin pain that worsens with flexion, adduction and medial rotation, potentially limitation of medial rotation or cracking. Pain in the sitting position;
• on the AP x-ray: non-spherical head, coxa vara, acetabular retroversion, coxa profunda;
• the Dunn view rules out offset, flat head-neck junction or tumefaction;
• CT-scan: retroversion of the superior portion of the acetabulum, minimal femoral anteversion;
• MRarthrogram: modification of the femur neck signal at the head-neck junction, rim lesion, modification of subchondral bone signal from the anterior portion of the acetabulum or the posterior horn.
