Résumé |
L’objectif des auteurs a été d’apprécier l’impact éventuel d’un prétraitement par omalizumab sur la tolérance et l’efficacité d’une immunothérapie spécifique (ITS). Sur la base d’un recueil d’expérience, il a été colligé 10 observations de patients ayant bénéficié d’une telle association. Six observations concernaient des patients sensibilisés au venin d’abeille, chez qui la progression de l’ITS fut interrompue en raison de problèmes de tolérance. Dans ces 6 observations, le prétraitement par omalizumab, selon des modalités variables, a été considéré comme efficace, permettant la mise en route et le maintien de l’ITS au venin d’abeille. Aucun essai clinique randomisé n’a évalué l’impact du traitement par omalizumab sur la tolérance d’une ITS aux venins d’hyménoptères, mais de nombreuses observations ont été publiées, soulignant l’efficacité d’un tel prétraitement pour améliorer la tolérance de cette ITS. Quatre observations concernaient des patients ayant un asthme allergique sévère, en rapport avec une sensibilisation aux acariens. Dans 3 de ces observations, l’association omalizumab et ITS aux acariens a été considérée comme efficace, en termes de contrôle de l’asthme ou de tolérance de l’ITS. Cette association ne fut pas efficace dans la quatrième observation. Ce recueil d’expérience et cette analyse de la littérature ont permis de mettre en avant le rôle d’un prétraitement par omalizumab, afin d’améliorer la tolérance d’une ITS au venin d’hyménoptère. Dans le contexte des allergies respiratoires, il existe des arguments pour une synergie entre ces deux modalités thérapeutiques, difficile à mettre en pratique notamment dans le cadre de l’asthme sévère.

Our aim was to assess the eventual impact of pretreatment with omalizumab on the tolerance and efficacy of specific immunotherapy (SIT). Based on a collection of experiences, we report observations on 10 groups of patients who have received that therapeutic combination. Six of these observations involved patients sensitized to bees; their SIT was stopped because of problems with tolerance. In these six observations, different sorts of pretreatment with omalizumab was considered effective, which allowed bee venom SIT to be started and then maintained. While no randomized clinical trial has evaluated the effect of omalizumab treatment on tolerance to hymenoptera venoms, a number of published reports emphasize the efficacy of this pretreatment in improving tolerance to this SIT. Four observations concerned patients with severe allergic asthma due to sensitization to mites. In three of these observations, the association of omalizumab and mite SIT was considered effective in terms of control of the asthma and tolerance to the SIT; the combination was not considered effective in the fourth case. This collection of observations and an analysis of the literature have allowed us to emphasize the role of pretreatment with omalizumab in improving tolerance to SIT with hymenoptera venom. In the context of respiratory allergies, there are arguments for the combination of these two therapeutic modes in spite of the difficulty of applying it, especially in the case of severe asthma.
