Résumé |
Objectifs |
Décrire les différentes techniques utilisées pour la circoncision des nouveau-nés et des nourrissons au bloc opératoire du CHU de Lomé (Togo) et comparer leurs résultats.
Patients et méthodes |
Il s’agit d’une étude prospective réalisée au bloc opératoire du CHU de Lomé du 15 juin 2007 au 15 décembre 2008, soit une période de 18 mois. Elle a concerné les nouveau-nés et nourrissons circoncis suivant deux techniques : celle utilisant uniquement des pinces (groupe A : n=138 ; 69 %) et celle utilisant le Gomco clamp (groupe B : n=62 ; 31 %). Le test Khi2 au seuil 5 % a été utilisé pour l’analyse statistique.
Résultats |
Selon la durée de l’intervention, 34 nouveau-nés et nourrissons (24,6 %) ont été circoncis dans un délai de moins de 15minutes dans le groupe A et 27 (43,5 %) dans le groupe B (p<0,05). En fonction de l’importance du saignement et selon le type de matériel utilisé, 28 nouveau-nés et nourrissons (20,30 %) avaient beaucoup saigné dans le groupe A et un (1,61 %) dans le groupe B (p<0,05). Selon les complications postopératoires et en fonction de la technique utilisée, neuf nouveau-nés et nourrissons (9,4 %) avaient des complications postopératoires dans le groupe A et deux (3,2 %) dans le groupe B. La plaie avait cicatrisé dans un délai de moins de six jours chez un nourrisson (0,7 %) dans le groupe A et six (9,7 %) dans le groupe B (p<0,05). Cent pénis (72,46 %) étaient jugés très satisfaisants dans le groupe A et 55 (88,7 %) dans le groupe B (p<0,05).
Conclusion |
La technique utilisant le Gomco clamp pour la circoncision avait beaucoup plus d’avantages que celle utilisant uniquement les pinces. Elle était beaucoup plus sécurisante pour l’enfant.

To describe the various techniques used for the circumcision in newborns and infants in the operating room of the Lomé Teaching Hospital (Togo) and to compare their results.
Patients and methods |
It is about a prospective study carried out in the operating room of the Lomé Teaching Hospital from June 15th, 2007 to December 15th, 2008 (18 months). It concerned newborns and infants circumcised according to two techniques: the technique using grips only (group A: n=138; 69%) and the technique using Gomco clamp (group B: n=62; 31%). The Khi2 test with the threshold 5% was used for the statistical analysis.
Results |
According to the duration of the intervention, 34 newborns and infants (24.6%) were circumcised within less than 15minutes in the group A and 27 (43.5%) in the group B (p<0.05). According to the importance of the bleeding and to the type of material used, 28 newborns and infants (20.30%) had bled much in the group A and one (1,61%) in the group B (p<0.05). According to the postoperative complications and to the technique used, nine newborns and infants (9.4%) had postoperational complications in the group A and two (3.2%) in the group B. The wound had healed within less than 6 days in one infant (0.7%) in the group A and six (9.7%) in the group B (p<0.05). One hundred penises (72.46%) were considered to be very satisfying in the group A and 55 (88.7%) in the group B (p<0.05).
Conclusion |
The technique using Gomco clamp for circumcision had more advantages than that using only the grips. It was much more safe for the child.
