Résumé |
Introduction |
L’imatinib (Glivec®) est un inhibiteur des tyrosines kinases utilisé dans certaines leucémies. Nous rapportons une observation de syndrome d’hypersensibilité médicamenteuse ou drug reaction with eosinophilia and systemic symptoms (DRESS) induit par l’imatinib.
Observation |
Une femme de 77 ans, atteinte d’une leucémie myéloïde chronique, a été traitée par de l’imatinib et de l’allopurinol. À j19, elle a développé une éruption cutanée polymorphe fébrile, avec des érosions buccales, associée à un œdème du visage, une polyadénopathie et une hyperéosinophilie sanguine. L’examen histologique cutané était en faveur d’une toxidermie. L’évolution a été favorable en 15 jours après l’arrêt des deux médicaments. Une réintroduction de l’imatinib trois mois après, nécessitée par l’évolutivité de la leucémie myéloïde chronique, a entraîné une récidive du DRESS dans les 12heures.
Discussion |
L’allopurinol a été arrêté définitivement devant une imputabilité plus fréquente. L’imatinib a été réintroduit après avoir considéré le rapport bénéfice/risque et en connaissance des effets cutanés secondaires fréquents, bien qu’un seul cas de DRESS ait été publié jusqu’alors. Selon les critères de Bégaud et al. l’imputabilité de l’imatinib, qui était plausible initialement (I2) chez cette malade, est devenue vraisemblable après l’épreuve de réintroduction (I3). Cette observation illustre le fait que l’imatinib est un médicament potentiellement inducteur de DRESS.

Background |
Imatinib (Glivec®) is a tyrosine kinase inhibitor used to treat certain cases of leukaemia. We report a case of a drug-induced reaction with eosinophilia and systemic symptoms (DRESS) caused by imatinib.
Case-report |
A 77-year-old woman with a chronic myeloid leukaemia was treated with imatinib and allopurinol. Nineteen days after the start of treatment, the patient presented fever with a generalized polymorphous rash associated with oral erosions, facial oedema, diffuse lymphadenopathy and blood hypereosinophilia. Histological analysis of skin biopsy specimens suggested a drug-induced reaction. The outcome was favourable two weeks after discontinuation of treatment. Three months later, imatinib was reintroduced because of progression of the patientʼs chronic myeloid leukaemia, and recurrence of the skin rash and fever was observed within 12hours.
Discussion |
Allopurinol was stopped definitively because of its more frequent imputability. Imatinib was reintroduced after considering the benefit-risk ratio and in full knowledge of the existence of cutaneous reactions to imatinib, despite there being only one recent report of DRESS following treatment with imatinib. According to the causality criteria of Bégaud et al. regarding imatinib, inherent causality of the drug in our patient was initially possible (I2) and appeared likely (I3) after the rechallenge test. This case clearly illustrates that imatinib is a potential cause of DRESS.
