A new perspective in psychiatry. From psychic totality to mental disorders multiplicity

Le trouble mental est habituellement conçu sous des formes définies à type de syndromes et d’entités relativement stables. Un renversement conceptuel consiste à passer de la description des formes apparentes à l’analyse de leurs infrastructures dynamiques cachées, à l’aide de nouveaux outils logico-mathématiques et d’une modélisation informatique constructiviste. On peut alors considérer le psychisme comme un système dynamique global auto-organisateur qui se modifie au contact du milieu extérieur. Animé par une énergie spécifique au patient, il dépend de lois qui contribuent à organiser de multiples structures pathologiques à la fois caractéristiques, mouvantes et transformables. Ses rapports avec les formes pathologiques sont envisagés en tenant compte d’une hiérarchie d’échelles spatio-temporelles différentes. Les faits cliniques corroborent ces diverses notions. Envisagés à partir des délires, ceux-ci en montrent le caractère global, la diversité structurelle, les mouvances, les oscillations, les transformations possibles, ainsi que les isomorphismes et homéomorphismes constitutifs répondant à une loi d’organisation. Une conclusion souligne le fait qu’un nouveau paradigme, renversant la perspective d’approche, offre des données nouvelles transformant le paysage psychiatrique.

The mental disorder is usually designed in very defined forms with type of relatively stable syndromes and entities. A conceptual reversal is to move from the description of apparent forms to the analysis of their hidden dynamic infrastructures, using new logical-mathematical tools and constructivist computer modeling. Then, we can consider the psyche as a comprehensive self-organizing dynamic system that changes in the contact with external environment. Driven by a specific energy of the patient, it depends of laws that help to organize multiple pathological structures that are characteristic, fluid and very changeable. The relationships of psyche with pathological forms are envisaged taking into account a hierarchy of different spatial and temporal scales. Clinical evidence supports these various concepts. Contemplated from deliriums, they show the global character, the structural diversity, the movements, the oscillations, the possible transformations and also the corresponding isomorphisms and homeomorphisms responding to an organizational law. The conclusion highlights the fact that a new paradigm, reversing the perspective of approach, provides new data transforming the mental and psychiatric landscape.
