Voir les affiliationsNous rapportons un cas de thrombopénie auto-immune associée à une réactivation virale B chez un malade initialement négatif pour l'antigène HBs et positif pour l'anticorps anti-HBs. La réactivation virale B est survenue 9 mois après une chimiothérapie comprenant un anticorps monoclonal anti-CD 20 suivie d'une autogreffe de moelle pour lymphome malin non hodgkinien. Après injection d'immunoglobulines polyvalentes et traitement par adéfovir dipivoxil, la thrombopénie s'est corrigée rapidement et la réactivation virale a été contrôlée avec séroconversion HBe et HBs en 5 mois. L'adéfovir a été arrêté 4 mois plus tard, sans rechute, avec un recul de quinze mois. Cette observation souligne l'importance de la surveillance des malades ayant été en contact avec le virus de l'hépatite B dans des situations d'immunosuppression, y compris lorsque les anticorps anti-HBs sont initialement présents.

We report a case of autoimmune thrombocytopenia associated with acute reverse seroconversion of hepatitis B in a patient who was initially hepatitis B virus surface antigen negative and hepatitis B virus surface antibody positive. Reactivation occurred 9 months after chemotherapy with anti-CD 20 monoclonal antibodies and autologous hematopoietic stem cell transplantation for lymphoma had been performed. After non specific polyglobulin injections and treatment with adefovir dipivoxil, thrombocytopenia and viral replication were controlled. Seroconversion for both HBe and HBs occurred at 5 months. Adefovir was stopped 4 months later with no relapse during fifteen months of follow-up. This case shows that patients who have had previous contact with hepatitis B virus should be monitored if they become immunosuppressed, even if anti-HBs were initially present.
