Single-compartment knee arthroplasty and idiopathic necrosis of the medial tibia surface |
Purpose of the study |
Idiopathic necrosis of the medial articular surface of the tibia is exceptional. Diagnosis is quite difficult and often made late. Among the different treatments proposed, we prefered single-compartment arthroplasty.
Material and methods |
We report 8 cases in women with a mean age of 71.1 years. Diagnosis was suspected due to drug-resistant knee pain, particularly frequent at night, initially with radiographically normal knees. The first radiographic signs, seen 3 months after the onset of pain, were pathognomonic for osteonecrosis evidencing subchondral defects of the tibial surface with a dense peripheral rim and apparently "sequestered" in a notch. Bone scintigraphy evidenced intense uptake in the medial compartment. MRI confirmed the diagnosis evidencing a band of low intensity signals completely surrounding a sequestered zone reaching the cortical. This band was stable and irreversible. In 5 cases CT scan and in 3 cases tomography identified the width and height of the necrotic area that was limited to the medial compartment in all cases. All patients were treated with a single compartment implant. The diagnosis of necrosis was confirmed at pathology.
Results |
At 4,6 years of mean follow up all patients had an excellent outcome, "forgetting" their knee. No lucent lines developed along the femoral or tibial implants.
Discussion |
Necrosis of the medial articular surface of the tibia is exceptional and often diagnosed late by bone scintigraphy or MRI. Surgical treatment is usually based on tibial osteotomy for valgisation or a single or three-compartment prosthesis. In our 8 cases, the necrosis was limited to the medial compartment, warranting our therapeutic option.
La nécrose idiopathique du plateau tibial interne est une affection rare, dont le diagnostic au début est difficile, volontiers retardé, et pour laquelle différents traitements ont pu être proposés. Notre choix a été celui de la prothèse unicompartimentaire.
Dans 8 observations chez des femmes d'âge moyen 71,1 ans, le diagnostic a été suspecté devant des gonalgies surtout nocturnes, rebelles à tout traitement avec radiographies initiales normales. Après un délai d'environ 3 mois sont apparus les signes radiologiques pathognomoniques de l'ostéonécrose associant géode sous-chondrale entourée d'une plage de densification du plateau tibial et au sein de la niche, une image pouvant faire évoquer un séquestre. La scintigraphie mettait en évidence une zone d'hyperfixation intense au niveau du plateau tibial interne. L'IRM confirmait le diagnostic en montrant un liseré se manifestant par une bande d'hyposignal entourant complètement un séquestre jusqu'à la zone corticale ; ce liseré avait pour caractéristique sa stabilité et son irréversibilité. Dans 5 cas un scanner et dans 3 cas une tomographie ont précisé les limites en largeur et en hauteur de la nécrose qui ne débordait jamais sur le plateau tibial externe. Tous les malades ont bénéficié d'une prothèse unicompartimentaire. L'anatomopathologie a toujours confirmé la nécrose.
Le résultat a été excellent, aucun liseré n'a été observé au contact de l'implant fémoral ou tibial au recul moyen de 4,6 ans.
Les nécroses du plateau tibial sont rares et de diagnostic retardé. Il est fondé sur la scintigraphie et l'IRM. Les traitements chirurgicaux font appel à l'ostéotomie tibiale de valgisation ou à la prothèse uni ou tricompartimentaire. Dans nos huit observations, le caractère limité au plateau tibial interne de la lésion, semble justifier notre option thérapeutique.
