Évaluer l'intérêt d'un test au Telebrix Gastro dans la prise en charge d'un syndrome obstructif du grêle et en particulier la valeur de ce test pour un traitement non chirurgical.

De façon prospective, de janvier 1995 à décembre 2000, 97 malades ont été admis pour occlusion du grêle sur bride. Tous ont bénéficié d'un test au Telebrix. Le test consistait en l'ingestion de 100 mL de Telebrix Gastro par la sonde naso-gastrique. Un abdomen sans préparation était effectué 4 et 8 heures après. Si le produit de contraste opacifiait le côlon sur le cliché réalisé 8 heures après, le test était considéré comme négatif pour l'occlusion.

Cent vingt-six cas d'occlusion intestinale sur bride ont été analysés en raison d'épisodes récurrents pour 11 malades. Le test a été négatif dans 113 cas (89,7 %) et parmi ces cas, seuls deux malades ont été opérés. Parmi les 10 malades dont le test était positif (10,3 %), tous furent opérés. La sensibilité, la spécificité et la pertinence d'un test négatif comme prédictif d'une évolution simple sous traitement conservateur étaient respectivement de 98 %, 100 % et 98 %.

Le test aux hydro-solubles, ici le Telebrix Gastro, est une aide pour la prise en charge des malades présentant une suspicion clinique d'occlusion sur bride.

This study aimed to determine whether an abdominal radiograph 8 hours after ingesting oral Telebrix Gastro is a reliable marker for non-operative management in patients with adhesive small bowel obstruction.

During a 5-year period (January 1, 1995, through December 31, 2000), 97 patients were admitted for small bowel occlusion due to adhesion with no indication for immediate surgery. All received 100 mL of Telebrix Gastro via gastric tube for small bowel obstruction due to adhesion. If the contrast reached the colon within 8 hours on plain abdominal radiograph, the test was considered to be negative.

126 cases of small bowel occlusions were analyzed due to recurring episodes for 11 patients. The test was negative in 113 cases (89.7%), and in this group, only two patients underwent surgery, the remaining being managed non-operatively. The 13 cases (10.3%) with a positive test for occlusion underwent surgery. The sensitivity, specificity and accuracy of the finding of contrast media reaching the colon as an indicator for conservative treatment were 98%, 100%, and 98%, respectively.

A water-soluble contrast study can be of significant help in the clinical management of patients suspected of having small bowel obstruction.
