La migraine ophtalmique, ou migraine avec aura (MA) visuelle, est caractérisée par la survenue récurrente de manifestations visuelles transitoires généralement suivie de céphalées. La physiopathologie est mal connue, mais pourrait être secondaire à l'apparition d'une onde de dépolarisation corticale responsable des manifestations visuelles transitoires, qui déclencherait un orage trigéminovasculaire responsable des céphalées. Le symptôme visuel le plus caractéristique est le scotome scintillant, sous forme d'une ligne brisée scintillante se déplaçant généralement progressivement du centre vers la périphérie du champ visuel, laissant dans son sillage un scotome. Les autres symptômes visuels vont de simples manifestations visuelles positives et/ou négatives élémentaires à de véritables hallucinations très élaborées. Le diagnostic repose sur les critères de l'International Classification of Headache Disorders (ICHD) dont la dernière révision date de 2013. Le traitement a pour principal objectif d'espacer et de soulager rapidement les crises douloureuses. Il n'y a pas de traitement spécifique actuellement de l'aura en elle-même.

Ophthalmic migraine, also called migraine with visual aura, is characterized by the recurrent episodes of transient visual manifestations usually followed by headache. The pathophysiology is poorly understood, but could be secondary to the occurrence of a cortical spreading depolarization wave causing transient visual manifestations, which would trigger a trigemino-vascular storm causing headache. The most characteristic symptom is the fortification spectrum, as a glittering broken line generally moving gradually from the center to the periphery of the visual field, leaving in its wake a scotoma. Other visual symptoms range from elementary simple visual manifestations, positive and/or negative, to true elaborate hallucinations. Diagnosis is based on the criteria of the International Classification of Headache Disorders (ICHD), which was last revised in 2013. The main goal of treatment is to reduce the frequency and quickly relieve painful attack. There is currently no specific treatment of the visual aura in itself.
