Traitement de l’hépatite chronique C chez les patients co-infectés VHC-VIH - 16/04/09

Chez les patients VIH co-infectés par le virus de l’hépatite C (VHC), la réponse au traitement anti-VHC était décevante dans les premiers essais princeps, avec au plus 44 % de réponse virale prolongée. Mais depuis, de nombreux enseignements ont été tirés de ces essais et notamment de l’essai ANRS HC 02 RIBAVIC, mais aussi des essais réalisés chez les patients mono-infectés par le VHC. Actuellement des résultats comparables aux patients mono-infectés par le VHC peuvent être obtenus chez les patients co-infectés VHC-VIH grâce à une meilleure gestion du traitement antirétroviral afin d’éviter un antagonisme entre le traitement anti-VHC et le traitement anti-VIH et d’améliorer la tolérance du traitement anti-VHC, une augmentation de la posologie de la ribavirine en cas de génotype 1 ou 4, une prolongation de la durée du traitement anti-VHC à 72 semaines voire plus en fonction des caractéristiques pré-thérapeutiques (âge > 40 ans, score METAVIR F3F4, charge virale VHC élevée), et de la réponse virale VHC à la semaine 4 et 12.

Since the pivotal trials conducted over the last few years which have used the combination of pegylated interferon along with fixed low doses (800mg/day) of ribavirin and have provided treatment for 12 months, regardless the kinetics of hepatitis C virus (HCV) viral load during therapy, great progress has been made in the treatment of chronic hepatitis C in HIV-infected patients. Results approaching those seen in HCV-monoinfected patients can be obtained with optimal dosages of ribavirin, extension of treatment beyond 48 weeks in the absence of rapid virologic response and/or in patients older than 40 years, with severe fibrosis (METAVIR score F3 or F4) and/or with high HCV viral load and with a better management of HIV treatment in order to avoid a negative interaction between HCV and HIV therapies (interaction between abacavir and ribavirin) and to improve the tolerance of HCV therapy (Didanosine should never be used with ribavirin, Zidovudine and Stavudine should be avoided when possible with ribavirin).
