Evaluer la proportion de malades toxicomanes suivis dans un centre spécialisé ambulatoire pouvant bénéficier d'un dépistage de l'infection par le virus de l'hépatite C (VHC) et l'efficacité de leur prise en charge.

Malades et méthodes

La réalisation d'une sérologie C était proposée systématiquement aux malades suivis dans un Centre de Soins Spécialisés pour Toxicomanes. Des explorations complémentaires et un traitement anti-viral si nécessaire étaient ensuite proposés en cas de sérologie VHC positive. Lorsqu'il existait une indication thérapeutique (score Métavir >= F2), le suivi était pratiqué conjointement par un médecin du centre et un hépatologue hospitalier.

Résultats

Entre le 01/07/1997 et le 30/09/2000, 404 malades (310 hommes, âge moyen : 32 ans, 51 % de consommateurs excessifs d'alcool, 94 % substitués pour leur toxicomanie) ont été inclus. La sérologie C a été réalisée dans 66 % des cas (269/404) : 84 % des malades testés avaient une sérologie C positive. Parmi ceux-ci, seulement 68 % ont effectué le dosage de l'activité des aminotransférases prescrit et 120 avaient une indication à la ponction biopsie hépatique. Quatre-vingt-huit biopsies hépatiques ont été réalisées : 22 % (20/88) des malades avaient une fibrose sévère (score Métavir F3 ou F4) et la consommation d'alcool était corrélée avec la sévérité histologique de la fibrose (P < 0,05). Le traitement anti-viral était indiqué chez 47 malades mais n'a pu être initié que chez 27 d'entre eux (refus du malade : n = 7 ; contre-indication : n = 13). Le traitement a dû être interrompu prématurément chez 12 malades, dans tous les cas pour raisons psychiatriques (dépression : n = 3 ; délire : n = 3 ; irritabilité majeure : n = 3 ; reprise de consommation de stupéfiants : n = 3). Finalement, seulement 5 malades étaient répondeurs prolongés.

Conclusion

Malgré une forte prévalence de l'infection par le VHC, le bénéfice du traitement anti-viral est faible dans cette population. La prise en charge de l'alcoolisme de ces malades doit être prioritaire.

Hepatitis C virus screening in drug users in an addiction out-patient unit

Aim

To investigate the feasibility and efficacy of hepatitis C virus screening in drug users in an addiction out-patient unit.

Patients and methods

All patients followed in an addiction out-patient unit were asked to undergo anti-hepatitis C virus antibody testing; further evaluation and treatment if indicated, were offered to positive patients. When treatment was initiated (Metavir score >= F2), patients were followed-up both by the hepatologist and the out-patient unit physician.

Results

Between July 1997 and September 2000, 404 consecutive patients (310 men, mean age: 32, alcohol intake >= 50 g per day in 51%, 94% in opiate substitution program) were included. Sixty-six per cent (269/404) of patients agreed to undergo HCV antibodies testing: 84% had a positive test. 68% of these patients accepted ALT serum measurement and 120 had indications for liver biopsy. Eighty-eight liver biopsies were performed, showing severe fibrosis (Metavir score F3 or F4) in 20 cases (22%). Ethanol intake was significantly correlated to fibrosis (P < 0.05). Antiviral treatment was indicated in 47 patients but was only initiated in 27 due to patient refusal (n = 7) or contraindication (n = 13). Treatment had to be discontinued in 12 cases because of psychiatric side effects (depression: n = 3; delirium: n = 3; severe irritability: n = 3; relapse with heroin injection: n = 3). Finally, only 5 patients were sustained responders.

Conclusion

Despite the high seroprevalence of HCV antibodies in this unit, the benefits of antiviral therapy are low due to high drop out rate. Ethanol withdrawal should be the highest priority in these patients.
