Objectif |
Le but de cette étude rétrospective était d'évaluer la performance de la scintigraphie péritonéale dans la mise en évidence d'une communication péritonéo-pleurale en cas de cirrhose compliquée d'hydrothorax et de discuter son apport dans la conduite thérapeutique.
Malades et méthodes |
Dix malades cirrhotiques avec un épanchement pleural ont été inclus. La cirrhose était secondaire à une hépatite virale dans 8 cas, auto-immune dans un cas et d'origine inconnue dans un cas. L'épanchement pleural était droit dans 9 cas et bilatéral dans un cas. Une scintigraphie péritonéale utilisant des sulfo-colloïdes marqués au technétium 99m a été réalisée chez tous les malades.
Résultats |
La scintigraphie a mis en évidence une communication péritonéo-pleurale dans 9 cas. Chez 4 malades une radioactivité pleurale était détectée quelques minutes après l'injection intra-péritonéale du radiotraceur. Chez 3 d'entre eux, une large brèche diaphragmatique était mise en évidence à l'échographie, à l'IRM ou à la thoracoscopie.
Le traitement médical a permis une résolution complète de l'hydrothorax chez 4 malades. Parmi les 4 malades chez qui un passage pleural très rapide de la radioactivité avait été noté, la résolution de l'hydrothorax par un traitement diurétique a été obtenue dans un seul cas. Parmi les 3 autres malades, avec hydrothorax réfractaire au traitement médical, un traitement par symphyse pleurale a été efficace dans un cas.
Conclusion |
La scintigraphie péritonéale est un examen simple et non invasif qui permet de confirmer l'existence d'une communication péritonéo-pleurale en cas d'hydrothorax au cours de la cirrhose. Elle permet également d'évaluer l'importance de la brèche diaphragmatique et, par conséquent, de contribuer à la stratégie thérapeutique.
Objective |
The aim of this retrospective study was to evaluate the performance of peritoneal scintigraphy for the diagnosis of peritoneopleural communication in patients with cirrhosis and to discuss its role in therapeutic management.
Patients and methods |
Ten patients with cirrhosis and pleural effusion were included in this study. Cirrhosis was due to viral hepatitis in eight patients, auto-immune disease in one patient and of unknown origin in one. The pleural effusion was right-sided in nine patients and bilateral in one. 99m-technetium sulfur colloid peritoneal scintigraphy was performed in all patients.
Results |
Scintigraphy revealed peritoneopleural communication in nine patients. In four patients, radioactivity appeared in the pleural cavity within a few minutes after injection of the radiotracer. In three of them, a large diaphragmatic defect was demonstrated by ultrasonography, magnetic resonance imaging or thoracoscopy. Complete response to medical treatment was observed in four patients. Scintigraphy revealed rapid radioactivity migration in four patients; diuretic treatment led to resolution of the hydrothorax in one of them. Three patients whose hydrothorax was refractory to medical treatment were treated by pleurodesis with talc. Resolution of the hydrothorax was achieved in one of them.
Conclusion |
Peritoneal scintigraphy is a simple non-invasive method enabling confirmation of peritoneopleural communication in cirrhotic patients. The importance of the diaphragmatic defect can also be evaluated, providing a significant contribution to therapeutic decision-making.
