La prise en charge chirurgicale de la douleur pancréatique requiert une neurectomie cœlioscopique tronculaire ou radiculaire des nerfs grands splanchniques (ngs). Le but de notre travail est de décrire le trajet et les rapports du ngs droit qui sont variables et peu décrits. Ceux-ci constituent un risque hémorragique peropératoire indéniable lors des splanchnicectomies.
Matériel et méthodes |
Après une double thoracotomie latérale et une désarticulation sterno-claviculaire bilatérale sur 15 sujets cadavériques adultes conservés par méthode de Winckler nous avons ôté le plastron sterno-costal ainsi que les arcs costaux ventraux et avons procédé à une éviscération médiastinale du thorax. Cette dernière ne laissait en place que l’aorte et l’œsophage thoraciques, le système veineux azygos, le conduit thoracique et les chaînes nerveuses sympathiques latéro-rachidiennes. Sur certains des 15 hémi-thorax droits disséqués, la veine azygos était injectée (après cathétérisme de sa crosse) à l’aide de gélatine colorée au bleu de peinture. Nous avons étudié le trajet et les rapports vasculaires des ngs droits. Nous avons mesuré les distances transversales entre l’origine du ngs, d’une part, et les axes longitudinaux de la veine azygos et du conduit thoracique, d’autre part.
Résultats |
Les rapports du tronc du ngs droit au cours de son trajet concernaient la veine azygos, en particulier son origine et ses affluents constitutifs : veine lombaire ascendante et douzième veine intercostale. Parfois le conduit thoracique voire un nœud lymphatique côtoyait le ngs dans l’espace infra-médiastinal postérieur. Une classification du trajet et des rapports vasculaires du ngs droit dans le thorax permettait d’identifier 3 types anatomiques. Les distances moyennes séparant le ngs, d’une part, et la veine azygos et le conduit thoracique, d’autre part, étaient respectivement de 5,7mm et de 11,2mm.
Conclusion |
Les rapports vasculaires du ngs droit sont très variables d’un sujet à l’autre mais essentiellement veineux, parfois lymphatiques. Il s’agit des gros vaisseaux péri-rachidiens du thorax dont le respect s’impose, notamment lors de l’abord mini-invasif cœlioscopique en vue d’une splanchnicectomie.

Aim |
The surgical assumption of responsibility of the pancreatic pain requires either a truncular coelioscopic or radicular neurectomy of greater splanchnic nerves (gsn). The goal of our work is to describe the way and relations of the right gsn which are variable and rarely described. This constitutes an undeniable peroperational hemorrhagic risk during splanchnicectomy.
Material and methods |
After a double side thoracotomy and a bilateral sterno-clavicular desarticulation on 15 adult cadaveric subjects preserved by method of Winckler we removed the sterno-costal drill plate as well as the ventral rib arch and proceeded to a mediastinal evisceration of the thorax. Then we respected only the thoracic aorta and the oesophagus, the azygos venous system, the thoracic duct and the thoracic sympathetic chain. In some of the subjects, the azygos vein was injected (after catheterization of its stick) using gelatine coloured with blue paint. We studied the way and vascular relations of the right gsn. We measured the transverse distances between the origin of the gsn on one hand and the longitudinal axes of the azygos vein and the thoracic duct on the other hand.
Results |
The relations of the right gsn trunk during its way related to the azygos vein in particular its constitutive origin and its affluents: ascending lumbar vein and twelfth intercostal vein. Sometimes the thoracic duct even a lymphatic node was near the gsn in the posterior infra-mediastinal space. A classification of the way and vascular relations of the right gsn in the thorax identified 3 anatomical types. The average distances separating the right gsn on one hand from the azygos vein and the thoracic duct on the other hand were respectively 5.7mm and 11.2mm.
Conclusion |
The vascular relations of the right gsn are very variable from one subject to another but primarily venous, sometimes lymphatic. They concerned the great thoracic vessels whose respect is essential in particular at the time of mini-invasive access procedure for a cœlioscopic splanchnicectomy.
