The experience of the EORTC anti-microbial group |
The context Up until the nineties, the intravenous administration of a broad spectrum antibiotic was the classical treatment of any patient presenting with febrile neutropenia. Since then, in patients considered at low risk and with expected of neutropenia less than 7-10 days, oral antibiotherapy has become an attractive option.
Two large studies A study by the antimicrobial group of the EORTC (European organisation for research and treatment of cancer) and a North American study have compared the efficacy of an oral combination of ciprofloxacine and amoxicillin/clavulanic acid with that of an intravenous antibiotherapy in low-risk patients presenting febrile neutropenia. In both studies, the success rate was the same in the group of patients treated with oral antibiotics and those treated with intravenous antibiotics.
Reservations These two studies were conducted in hospitalised patients. No conclusions can be drawn with regard to out-patient treatment. Out-patient management would only be possible after appropriate selection of patients at low risk.
Le contexte L'administration intraveineuse d'un antibiotique à large spectre constituait, jusqu'au milieu des années 90, le traitement classique de l'état fébrile chez tout patient neutropénique. Depuis, chez les patients considérés comme à bas risque avec notamment une neutropénie attendue inférieure à 7-10 jours, l'antibiothérapie orale est devenue une option attractive.
Deux grandes études Une étude du groupe antimicrobien de l'EORTC (European organisation for research and treatment of cancer) et une étude américaine ont comparé l'efficacité d'une combinaison orale de ciprofloxacine et d'amoxicilline/acide clavulanique à celle d'une antibiothérapie intraveineuse chez des patients neutropéniques fébriles à bas risque. Dans ces deux études, le poucentage de succès a été le même dans le groupe d'antibiothérapie orale et dans celui d'antibiothérapie intraveineuse.
Réserves Ces deux études ont été réalisées chez des patients hospitalisés. Elles ne permettent aucune conclusion concernant un traitement ambulatoire. Une prise en charge extra-hospitalière n'est possible qu'après une sélection appropriée des patients à bas risque.
