Introduction Les rhinorrhées par fistule cérébro-spinale sont l’apanage de l’évolution traitée des macroadénomes hypophysaires. Nous rapportons une situation inhabituelle de rhinorrhée survenant chez une femme atteinte d’un syndrome de Sheehan.
Observation Une patiente de 39 ans avait un hypopituitarisme complet évoluant depuis 13 ans. Le traitement hormonal substitutif s’était accompagné de la survenue d’un syndrome métabolique associant une obésité, un diabète de type 2, une hypertension artérielle et une hyperlipidémie mixte. La rhinorrhée a été confirmée par le dosage du glucose dans le liquide et rattachée par l’IRM cérébrale à une fistule ethmoïdale favorisée par une arachnoïdocèle intra-sellaire. L’enquête étiologique est restée négative, avec en particulier une absence de traumatisme et une ostéodensitométrie normale, rendant peu probable un défect osseux. Par contre, le terrain métabolique et en particulier l’obésité suggéraient un mécanisme primitif de la selle turcique vide.
Commentaires La selle turcique vide survenant au cours du traitement du syndrome de Sheehan peut être à l’origine de rhinorrhée. La responsabilité de l’obésité doit être étayée par d’autres travaux.
Nontraumatic ethmoid cerebrospinal fluid rhinorrhea complicating Sheehan syndrome |
Introduction Treatment of pituitary macroadenoma frequently leads to nontraumatic cerebrospinal fistula and rhinorrhea. We report an unusual case of rhinorrhea in a woman with Sheehan’s syndrome.
Case A 39-year-old woman receiving hormone replacement therapy for hypothyroidism diagnosed 13 years earlier (Sheehan’s syndrome) developed metabolic syndrome, which combining obesity, type 2 diabetes, hypertension and mixed hyperlipidemia. Cerebrospinal fluid rhinorrhea was confirmed by measurement of the glucose concentration in her nasal discharge and by cerebral MRI, which revealed a fistula of the ethmoid bone and an empty sella. Etiological screening was negative, and normal bone densitometry as well as the absence of trauma ruled out any bone defects. Conversely, the metabolic syndrome, notably obesity, suggested a nontraumatic mechanism for the empty sella.
Discussion Onset of empty sella syndrome during treatment for Sheehan’s syndrome may be the cause of the CSF rhinorrhea. The role of obesity requires further investigation.
