Résumé

Objectifs

Gaetano Benedetti, psychanalyste d’origine italienne fut Professeur de psychiatrie à Bâle après s’être formé auprès de Manfred Bleuler, Margueritte Sechehaye, Harry Stack Sullivan, John N. Rosen, entre autres. Son œuvre, internationalement reconnue, est essentiellement tournée vers la conception de repères, de modèles, de notions propres à rendre compte et à soutenir la relation psychothérapeutique avec les personnes schizophrènes. L’originalité de son approche repose sur la nature de l’engagement du thérapeute dont les mouvements transférentiels et inconscients sont autant d’outils pouvant permettre à celui-ci d’accéder et d’œuvrer à transformer la réalité délirante ou autistique de son patient.

Méthode

Simultanément à la description du parcours de Benedetti qui, en tant que clinicien, rencontra de multiples écueils, les principaux concepts cliniques qu’il élabora seront ici exposés et illustrés d’exemples cliniques.

Résultat

Toute la rigueur de la conceptualisation de G. Benedetti ne vise point à se défaire d’une clinique exigeante et difficile, mais au contraire de s’y engager au-delà de ce que les pratiques orthodoxes ne le permettent. Il conçoit à cet effet des outils permettant de penser non pas tant une technique qu’une forme de disposition d’esprit et une façon d’être avec la psychose. La « positivation » de la psychopathologie, par exemple, relève d’un mouvement double par lequel le thérapeute reconnaît les expériences psychotiques comme autant de tentatives désespérées face à l’effondrement qui menace, sans chercher à les confronter au principe de réalité mais en s’y « identifiant partiellement », parfois à la frontière d’une « folie à deux », laissant à l’inconscient du patient comme du thérapeute l’opportunité de trouver une « ouverture ». Patient et thérapeute élaborent de concert par associations conjointes, le thérapeute pouvant mettre à disposition des « images transformantes » qui, par l’effet d’une « dualisation » et sous l’effet de ce que Benedetti appelle le « sujet transitionnel », permettent une forme de partage, rudimentaire peut-être, mais fondamental pour un sujet qui ne peut survivre que dans le plus extrême retrait. Le thérapeute ainsi peut, par exemple, raconter ses propres rêves à son patient, s’ils concernent bien sûr ce dernier, car ils peuvent s’avérer résolutifs de conflits dans lesquels le patient se trouve englué.

Discussion

La folie telle que conçue par Benedetti est irréductible aux seuls concepts car son traitement nécessite que le thérapeute se mette subjectivement en jeu et partage une expérience fondamentale avec son patient. Ce partage permet, selon Benedetti, l’élaboration conjointe de symboles, véritables opérateurs psychiques, qui n’étaient jamais venu à se structurer ou bien que le processus psychotique avait disjoint.

Conclusion

La démarche analytique de Benedetti s’oppose points par points à une entreprise archéologique d’investigation mais se rapproche plutôt d’un travail de construction où l’engagement, l’implication du thérapeute visent essentiellement à rejoindre un sujet que la psychose isole radicalement. L’élaboration théorique permanente et le travail essentiel de la supervision offrent un appui précieux et essentiel pour soutenir une démarche souvent périlleuse.

Abstract

Objectives

Gaetano Benedetti, a psychoanalyst of Italian origin, was Professor of psychiatry in Basel, having previously trained with Manfred Bleuler, Marguerite Sechehaye, Harry Stack Sullivan, and John N. Rosen, among others. His work, internationally recognised, essentially centres around the concept of markers, models and distinct notions to explain and support the psychotherapeutic relationship with schizophrenics. The originality of his approach lies in the nature of the commitment of the therapist, whose transferential and unconscious movements become tools enabling the therapist to gain access and work on transforming the delirious or autistic reality of his patient.

Method

The main clinical concepts that Beneditti elaborated will be set out here and illustrated with clinical examples, at the same time describing his itinerary as a clinician, and the many stumbling blocks.

Result

The stringency of Benedetti's conceptualisation is not designed to escape a demanding and difficult practice; on the contrary, it aims to engage beyond which orthodox practice allows. He conceived of tools for this purpose enabling reflection not so much on a technique as on a type of mind-set and a way of being/living with psychosis. The “positivisation” of psychopathology, for example, derives from a dual movement whereby the therapist recognises psychotic experiences as desperate attempts in the face of threatening collapse, without looking to confront them with the reality principle, but “partially identifying” with them, sometimes on the verge of “shared madness”, affording the patient's unconscious, as well as that of the therapist, the opportunity to find an “opening”. Patient and therapist elaborate together through shared associations; the therapist is able to supply “transforming images” which, together with “dualisation” and the effect of what Benedetti calls “the transitional subject”, enable a form of sharing, perhaps basic, but fundamental for a subject who can only survive in the most extreme withdrawal. Thus the therapist can, for example, recount his own dreams to the patient, if of course they are relevant, since they may solve conflicts in which the patient finds himself trapped.

Discussion

Madness as conceived by Benedetti cannot be reduced to mere concepts, since its treatment requires the therapist to become involved subjectively and to share an essential experience with his patient. According to Benedetti, this sharing enables the joint elaboration of symbols, genuine psychic operators that have never been structured, or that the psychotic process has broken down.

Conclusion

Benedetti's analytical approach argues point by point against an investigative “archaeological” undertaking, drawing closer to a work of construction where commitment and involvement of the therapist aim essentially to meet with a subject that the psychosis has radically isolated. The on-going theoretical elaboration and the essential supervision offer precious and vital support to back up an approach that is often fraught with risk.
