Introduction Les nocardioses sont des infections rares, généralement systémiques, survenant chez le sujet immunodéprimé. Nous rapportons un cas d’abcès cérébral à Nocardia farcinica chez une patiente immunocompétente, avec association inhabituelle à une embolie pulmonaire.
Observation Il s’agissait d’une femme âgée de 62 ans, sans antécédents médicaux, hospitalisée pour une crise comitiale de type Bravais-Jacksonienne. Le tableau clinique et radiologique était peu spécifique. Le diagnostic a été posé sur l’identification du micro-organisme. L’évolution a été favorable après exérèse de l’abcès par craniotomie et antibiothérapie adaptée pendant 4 mois.
Commentaires L’espèce N. farcinica se distingue des autres espèces par son haut degré de résistance aux antibiotiques et sa grande virulence. Nous insistons sur la nécessité d’une identification, non seulement du micro-organisme mais aussi de l’espèce en cause, car elle peut influer sur la conduite thérapeutique.

Introduction Nocardiosis is a rare, generally systemic infection that occurs in immunocompromised patients. We report a case of Nocardia farcinica primary brain abscess in an immunocompetent patient, unusually associated with a pulmonary embolism.
Observation A 62 year-old woman without medical past was hospitalised because of a Bravais-Jacksonian type of seizure. The clinical and radiological profile was unspecific. The diagnosis was made on identification of the microorganism. The patient improved after craniotomy with excision of the abscess and appropriate antibiotic therapy for four months.
Discussion The N. farcinica species is distinct from others by its high degree of antibiotic resistance its virulence. We insist on the need to identify not only the microorganism but also the species concerned because this may influence the treatment strategy.
