Résumé |
Introduction |
L’identification du statut RAS (KRAS et NRAS) a modifié la prise en charge des cancers colorectaux métastatiques (CCRm). L’impact de la mutation de RAS sur l’efficacité des chimiothérapies cytotoxiques n’est à ce jour pas déterminé. Certaines études rétrospectives seraient en faveur d’une plus grande efficacité de l’oxaliplatine dans les CCRm avec mutation de KRAS.
Méthodes |
Il s’agit d’une étude rétrospective évaluant les médianes de survie sans progression (SSP) et de survie globale (SG) d’une population de 216 patients atteints d’un CCRm et traités entre 2008 et 2010 par une chimiothérapie de 1re ligne avec oxaliplatine ou irinotécan, en association à une fluoropyrimidine et au bévacizumab.
Résultats |
Lorsque la tumeur était KRAS muté, la médiane de SG était de 23,4 mois avec le doublet à base d’oxaliplatine et de 23,6 mois avec le doublet à base d’irinotécan, sans différence significative (HR 1,30 ; 95 % IC 0,81–2,09 ; p=0,27). Lorsque la tumeur était KRAS sauvage, la médiane de SG était de 30,3 mois avec le doublet à base d’oxaliplatine et de 25,4 mois avec le doublet à base d’irinotécan, sans différence significative (HR 0,81 ; 95 % IC 0,52–1,24 ; p=0,33). Le statut KRAS n’avait pas non plus d’impact significatif en SSP sur l’efficacité de l’oxaliplatine ou de l’irinotécan.
Discussion |
Dans cette étude rétrospective, le statut KRAS n’influence pas l’efficacité de l’irinotécan ou de l’oxaliplatine, en association au bévacizumab, en première ligne de traitement des CCRm.

Introduction |
The identification of RAS status (KRAS and NRAS) has changed the management of metastatic colorectal cancer (mCRC). The impact of the RAS mutation on cytotoxic chemotherapy efficacy has not yet been determined. Nevertheless, several retrospective studies suggest a greater efficacy of oxaliplatin in mCRC with KRAS mutation.
Methods |
We analyzed retrospectively the progression free survival (PFS) and overall survival (OS) in 216 patients with mCRC receiving first line treatment between 2008 and 2010 according to KRAS status and chemotherapy regimen used (oxaliplatin- or irinotecan-based, in association with fluoropyrimidine and bevacizumab).
Results |
In KRAS mutated tumors, median OS was 23.4 months with oxaliplatin-based regimen, and 23.6 months with irinotecan-based regimen, with no significant difference (HR 1.30; 95 % CI 0.81–2.09; P=0.27). In KRAS wild-type tumors, median OS was 30.3 months with oxaliplatin-based regimen, and 25.4 months with irinotecan-based regimen, with no significant difference (HR 0.81; 95 % CI 0.52–1.24; P=0.33). Similarly, KRAS mutational status had no significant effect on efficacy of oxaliplatin or irinotecan regarding PFS.
Discussion |
In this retrospective study, KRAS mutational status does not influence the efficacy of first line cytotoxic chemotherapy in association with bevacizumab.
