Résumé |
L’hépatite cytolytique immunoallergique est une complication rare des agents halogénés. Une patiente de 37ans, sans antécédents particuliers, bénéficiait d’un curetage pour fausse couche spontanée sous anesthésie générale, réalisée sous sévoflurane. À j2 postopératoire apparaissaient des douleurs abdominales, un pic hyperthermique isolé et un ictère. Une insuffisance hépatique (IH) grave était diagnostiquée à j10. L’implication du sévoflurane a été évoquée sur la négativité des autres causes d’IH et sur la biopsie hépatique retrouvant une hépatite aiguë sévère avec nécrose en pont. Une transplantation hépatique (TH) a été réalisée face à l’aggravation clinique et biologique de cette hépatite et à l’échec de la corticothérapie. L’analyse de la pièce d’hépatectomie confirmait la nécrose hépatocytaire multilobulaire massive, compatible avec l’étiologie toxique. La patiente décédait 2 mois après la TH dans un tableau de syndrome de défaillance multiviscérale sur un choc septique sans porte d’entrée identifiée. Même si l’hépatite cytolytique immunoallergique au sévoflurane est théoriquement peu probable, ce cas clinique et d’autres dans la littérature tendent à démontrer ce risque de toxicité dont la physiopathologie n’est pas élucidée.

Immunoallergic cytolytic hepatitis is a rare adverse event of volatile anesthetic agents. A 37-year-old woman with no previous medical or surgical history underwent uterine curettage for spontaneous miscarriage under general anesthesia with sevoflurane. Two days after surgery, she developed abdominal pain, fever and jaundice. On the tenth postoperative day, laboratory tests revealed liver failure (LF). Severe sevoflurane hepatotoxicity was proposed due to the absence of any other causes for LF and to the presence of acute hepatitis with bridging pattern at the hepatic biopsy. The patient's clinical and biological condition worsened despite steroid therapy leading to emergency liver transplantation (LT). The liver's pathology examination observed acute hepatitis with massive multilobular hepatocellular necrosis, compatible with a toxic etiology. The patient died two months after LT from a multiple organ failure triggered by septic shock of undetermined origin. Immunoallergic cytolytic hepatitis is theoretically not caused by sevoflurane. This case report and others in the literature tend to demonstrate the existence of this toxicity although the underlying pathophysiology remains unclear.
