Évaluation de l'antigénémie aspergillaire dans la surveillance des patients neutropéniques - 04/03/08

La recherche des antigènes circulants tant dans le diagnostic de l'aspergillose invasive (AI) que dans la surveillance des patients à risque est devenue un élément important du diagnostic biologique de l'AI.
Un protocole de surveillance par dosage de l'antigénémie aspergillaire (AA) a été établi au Centre de Greffe de Moelle osseuse de Tunis de mars 2000 à septembre 2001 incluant tous les patients neutropéniques hospitalisés pendant cette période. Soixante quatorze patients ont été ainsi inclus ; chacun a bénéficié de prélèvements hebdomadaires (deux/semaine) pour la recherche de l'AA par méthode immunoenzymatique (Platelia® Aspergillus ).
Parmi ces patients, sept ont présenté des AI dont quatre aspergilloses probables avec isolement d' Aspergillus fumigatus dans le liquide de lavage broncho-alvéolaire dans un cas, deux aspergilloses possibles et une aspergillose non classée. L'AA était positive chez cinq de ces patients (durant plusieurs semaines consécutives) et négative chez deux. Par ailleurs, l'AA était faussement positive chez cinq autres patients ne présentant aucun signe clinique ou radiologique évocateur d'AI ; elle l'était de façon transitoire (pendant une semaine, voire deux).
Ce travail permet ainsi de dire qu'une AA positive aide certainement au diagnostic de l'AI (sensibilité = 71 % ; valeur prédictive positive = 50 %), notamment quand elle l'est pendant plusieurs semaines de suite, permettant ainsi d'instaurer rapidement un traitement antifongique. Par contre, une AA négative permet pratiquement d'exclure ce diagnostic (spécificité = 92 % ; valeur prédictive négative = 97 %).
Evaluation of aspergillary antigenemia for monitoring neutropenic patients |
The research of antigens circulating in the diagnosis of the invasive aspergillosis (IA) as well as in the control of patients who might be contaminated, has become an important factor of the biological diagnosis of the IA.
Formalities of control by means of measuring out the aspergillary antigenemia (AA) have been established in the centre of bone marrow transplant in Tunis from March 2000 to September 2001 including all neutropenic patients admitted in hospital during that laps of time.
74 patients have been grouped in such a way. Each of them has had a weekly taking of samples of blood (two per week) in order to research the AA in an immunoenzymatic way (Platelia® Aspergillus ).
Among those patients, seven have shown IA four of which are probable with identification of Aspergillus fumigatus in the liquid of broncho-alveolar washing in one case. Two other IA are possible and one in unclassified. The AA was positive for five of those patients (during several subsequent weeks) and negative for the two others.
Otherwise, the AA was positive for five other patients showing no clinical or radiological symptom revealing IA; it was present only in a transitory way (during one week even two).
Consequently, this task allows us to state that a positive AA is surely of a great help for the diagnosis of the IA (sensitivity = 71% predictive positive value = 50%) mainly when it is positive during several subsequent weeks, thus allowing to start a quick and efficient antifungal treatment. In contrast, a negative AA, practically, permits to exclude this diagnosis (specificity = 92% predictive negative value = 97%).
