Nous rapportons le cas d'un patient de 52 ans infecté par le VIH, au stade SIDA avec une rétinite bilatérale à CMV, traitée par ganciclovir (cymevan ® ) puis par du cidofovir. Au cours du traitement d'attaque, le patient se plaint d'une baisse d'acuité visuelle majeure bilatérale. Un traitement ophtalmologique local associant anti-inflammatoire stéroïdien et cycloplégiques est institué. Devant la sévérité du tableau et la non réponse au traitement local, on décide l'arrêt du cidofovir relayé par du foscarnet (foscavir ® ). L'inflammation disparaît mais l'arrêt de secrétion ciliaire persiste toujours avec un recul de 12 mois. Le patient présente une hypermétropisation majeure de + 11 avec un oedème choroïdien important et l'installation progressive d'une cataracte bilatérale. Cette observation est originale par la bilatéralité de l'uvéite et surtout par le caractère permanent de l'hypotonie. La revue de la littérature retrouve 3 % d'hypotonie définitive due au cidofovir en administration systémique. On peut émettre l'hypothèse d'une nécrose bilatérale du corps ciliaire d'origine iatrogène comme cela a été décrit sur des modèles animaux.

We report the case of a 52-year-old man with AIDS and CMV bilateral retinitis, treated first with ganciclovir (cymevan ® ) and then with cidofovir. During the treatment, the patient complained of a sharp decrease in visual acuity. Cycloplegics and topical corticosteroids were given. Then cidofovir was stopped and replaced with intravenous Foscavir because of the clinical inefficacy. The anterior uveitis resolved but the ciliary secretion has not stopped after 12 months of follow-up. The patient presented +11 hypermetropia with a choroidal edema and bilateral cataract. This case is interesting for its permanent hypotony and bilateral uveitis. A literature search has brought out a rate of only 3% of chronic hypotony.
We hypothesize bilateral iatrogenic ciliary body necrosis, as described in the animal models.
