But : L’objectif de cette étude est de déterminer les rapports anatomiques des implants phaques de chambre postérieure (IPCP) de modèles ICL et PRL par échographie de très haute fréquence (ETHF).
Matériels et méthodes : Nous avons examiné rétrospectivement par ETHF 17 yeux corrigés par ICL et 14 yeux après mise en place d’un PRL. Les principaux paramètres mesurés et comparés étaient la profondeur de chambre antérieure, la distance au centre et en périphérie entre l’IPCP et le cristallin, et la position exacte des haptiques.
Résultats : Les distances moyennes entre l’endothélium cornéen et l’IPCP étaient de 2 398 ± 203 µm et de 2 640 ± 230 µm dans les groupes « ICL » et « PRL » respectivement. L’espace central préservé entre l’implant et le cristallin était plus important dans les yeux du groupe « ICL » (402 ± 194 µm dans le groupe « ICL », 256 ± 187 µm dans le groupe « PRL », p ≪ 0,05). Cependant, l’incidence des contacts avec le cristallin en périphérie était plus élevée dans le groupe « ICL » (41 %) que dans le groupe « PRL » (29 %), et la distance entre le cristallin et l’IPCP en périphérie était statistiquement différente entre les deux implants (p ≪ 0,05). Les haptiques étaient correctement positionnées dans le sulcus pour 13 yeux (76 %) du groupe « ICL », et sur la zonule pour 8 yeux (57 %) du groupe « PRL ».
Conclusion : L’implantation phaque de chambre postérieure est une procédure sûre au regard des résultats réfractifs immédiats. L’ETHF offre un moyen unique d’évaluer les relations de ces implants au sein de la chambre postérieure, afin de mieux comprendre les complications cristalliniennes et iriennes.
Ultrasound biomicroscopy of posterior chamber phakic intraocular lenses: a comparative study between ICL and PRL models |
Purpose: To evaluate the anatomic relationships of the implantable contact lens (ICL) and the phakic refractive lens (PRL) posterior chamber phakic intraocular lenses (PCP IOL) using ultrasound biomicroscopy (UBM).
Material and methods: Seventeen phakic myopic eyes corrected with ICL, and 14 phakic myopic eyes that had had PRL implantation, were examined retrospectively using UBM. The main parameters measured and compared were anterior chamber depth, central and peripheral distance between PCP IOL and the crystalline lens, and exact lens haptic position.
Results: The mean distance between the PCP IOL and the central endothelium was 2398±203 µm and 2640±230 µm in the ICL and PRL groups, respectively. The central vault between the implant and the crystalline lens was greater in eyes with ICL (ICL, 402±194 µm; PRL, 256±187 µm, p≪0.05). However, the incidence of lens contact on the peripheral level was higher in the ICL group (41%) than in the PRL group (29%), and the difference between the two implants in the peripheral crystalline lens–PCP IOL distance was significant (p≪0.05). Both IOL haptics appeared to be correctly positioned in the sulcus in 13 (76%) eyes of the ICL group, and on the zonule in eight eyes (57%) of the PRL group.
Conclusions: PCP IOL implantation is a safe procedure for the correction of high myopia with regard to refractive results. UBM provides a unique tool to noninvasively evaluate the relations of these implants within the posterior chamber, and helps to analyze the mechanisms of crystalline lens and iris complications.
