Introduction : L’hamartome combiné de l’épithélium pigmentaire et de la rétine est une affection rare, caractérisée par une prolifération de l’épithélium pigmentaire rétinien et une gliose rétinienne entraînant une désorganisation de la rétine et de la papille. Le but de ce travail est de présenter l’intérêt d’un diagnostic précoce et d’une surveillance régulière.
Observations : Nous rapportons le cas de quatre enfants suivis entre 2001 et 2004 présentant un hamartome combiné de l’épithélium pigmentaire et de la rétine découvert entre l’âge de 3 mois et celui de 8 ans. Le motif de consultation était à chaque fois une baisse de l’acuité visuelle. L’examen clinique montrait l’existence d’une formation péripapillaire légèrement grisâtre avec des vaisseaux rétiniens tortueux. Cette affection fut confirmée par l’angiographie en fluorescence dans trois cas et par la tomographie en cohérence optique (OCT) dans un cas. L’évolution a été marquée par la persistance d’une acuité visuelle basse dans les quatre cas et l’apparition d’une membrane néovasculaire dans un cas.
Discussion : L’hamartome combiné de l’épithélium pigmentaire et de la rétine est une tumeur vraisemblablement congénitale, dont la pathogénie n’est pas encore élucidée. Le diagnostic est clinique et confirmé par l’angiographie rétinienne et l’OCT. Dans cette affection, il est important d’éliminer un rétinoblastome et un mélanome malin de la choroïde, d’où l’intérêt d’une exploration radiologique. L’évolution est stationnaire, néanmoins une baisse de l’acuité visuelle peut être en rapport avec une membrane épirétinienne ou une membrane néovasculaire.
Conclusion : La connaissance de l’aspect clinique s’impose pour différencier cette affection des processus malins rétiniens. C’est une tumeur qui peut être évolutive malgré son caractère bénin. Un suivi régulier est capital car il permet d’améliorer le pronostic visuel.

Introduction: Combined hamartoma of the retina and retinal pigment epithelium is a rare condition, characterized by a proliferation of the retinal pigmentary epithelium and retinal gliosis leading to a disorganization of the retina and papilla. This study aimed to demonstrate the advantages of early diagnosis and regular monitoring.
Observations: We report a series of four children followed between 2001 and 2004 with combined hamartoma of the retina and retinal pigment epithelium, with age of diagnosis ranging from 3 months to 8 years. The main reason for consultation was reduction of vision. The clinical examination objectified the existence of a slightly grayish peripapillary formation with tortuous retinal vessels. This condition was confirmed by angiography with fluorescein in three cases and by optical coherence tomography (OCT) in one case. Progression showed the persistence of low vision in all four cases and the appearance of a neovascular membrane in one case.
Discussion: Combined hamartoma of the retina and retinal pigment epithelium is probably a congenital tumor whose pathogenesis has not yet been elucidated. The diagnosis is clinical and the patient can be thoroughly examined by retinal angiography and optical coherence tomography (OCT). In this disorder, it is important to eliminate retinoblastoma and malignant melanoma of the choroid, showing the advantage of radiological exploration. Progression is stationary; nevertheless a reduction in visual acuity can be related to an epiretinal membrane or a neovascular membrane.
Conclusion: Knowledge of the clinical aspect is essential to differentiate this condition from the malignant retinal processes. This tumor can progress in spite of its benign character. Regular follow-up is essential and can improve the visual prognosis.
