Le syndrome d’Ondine ou Congenital Central Hypoventilation Syndrome (CCHS) appartient aux neurocristopathies (anomalie de migration ou de différentiation des cellules de la crête neurale). Il se caractérise par des hypoventilations ou des apnées survenant préférentiellement pendant le sommeil, en dehors de toute atteinte neuro-pulmonaire. La détresse respiratoire dès la naissance nécessite intubation et ventilation pour une période durable. Il s’y associe volontiers d’autres localisations ou manifestations des neurocristopathies (syndrome de Hirschsprung, neuroblastomes, neurogangliomes) et d’autres atteintes du système nerveux autonome (troubles vasomoteurs ou anomalies ophtalmologiques : anomalies pupillaires, insuffisances de convergence, strabisme ou ptôsis).
Nous rapportons le cas d’un patient atteint de CCHS présentant un ptôsis alternant de l’œil droit et de l’œil gauche, ainsi qu’une ésotropie précoce. Les signes oculaires doivent permettre de poser un diagnostic précoce et une prise en charge adaptée plus rapide.

Ondine’s Curse or congenital central hypoventilation syndrome (CCHS) is a neurocristopathy (failure of migration or differentiation of neural crest-derived precursor cells) and is characterized by hypoventilation or apnea, which is most pronounced during sleep, with no other abnormalities of the neuro-respiratory system. Because of respiratory distress soon after birth, patients must be intubated and ventilated for a long time. This disorder may be associated with other symptoms of neurocristopathy (Hirschsprung disease, neuroblastoma, neuroganglioma) and other abnormalities of the autonomic nervous system (vasomotor dysfunctions or ophthalmic abnormalities: abnormal pupils, insufficient convergence, strabismus, or ptosis). We report the original case of a CCHS patient who presented with alternative ptosis of both the right and left eyes and esotropia. The ocular findings should lead to earlier diagnosis and speedier adequate treatment.
