Traitement des perforations d'ulcère gastroduodénal par suture simple suivie de l'éradication de Helicobacter pylori - 01/01/02

Buts de l'étude : Apprécier la prévalence de l'infection par Helicobacter pylori (HP) au Vietnam en cas de perforation d'ulcère gastroduodénal et évaluer l'efficacité du traitement médical sur l'éradication d'HP et la cicatrisation de l'ulcère.
Patients et méthodes : Cent onze patients ayant une perforation d'ulcère gastroduodénal ont été traités par suture simple. Le diagnostic de l'infection par HP a été basé sur le test à l'uréase et l'examen histopathologique de biopsies antrales peropératoires et enfin par la recherche postopératoire des immunoglobulines G anti-HP. L'infection par HP était définie par au moins un résultat positif. Le traitement médical consistait en une trithérapie faite d'oméprazole, de clarithromycine, et d'amoxicilline pendant sept jours, suivie par un traitement par oméprazole seul pendant trois semaines en cas d'ulcère duodénal ou cinq semaines en cas d'ulcère gastrique. Un contrôle endoscopique avec biopsies était effectué quatre à six semaines après la fin du traitement.
Résultats : Sur les 111 patients, 107 (96 %) étaient infectés par HP. Les quatre patients non infectés par HP ont été traités par oméprazole seul et ont été exclus de l'étude. L'ulcère a cicatrisé chez 99 patients (93 %). L'éradication d'HP a été obtenue chez 102 patients (95 %). Les huit patients ayant un ulcère non cicatrisé et les cinq patients pour lesquels l'éradication n'était pas obtenue au premier contrôle endoscopique ont reçu un traitement médical supplémentaire. En définitive, l'éradication et la cicatrisation ont été constatées dans tous les cas.
Conclusion : En cas de forte prévalence de l'infection à HP, il n'est pas nécessaire d'associer un geste réduisant l'acidité gastrique à la suture d'un ulcère gastroduodénal perforé.

Aims of the study: To determine the prevalence of Helicobacter pylori (HP) infection in Vietnam in case of perforated gastro-duodenal ulcer and to evaluate efficiency of medical treatment to obtain eradication of HP and ulcer healing.
Patients and methods: One hundred and eleven patients with perforated gastro-duodenal ulcer underwent surgical suture. HP infection was diagnosed by urease test and pathologic examination of peroperative antral biopsies, and by postoperative detection of HP-specific immunoglobulins G. HP infection was defined by positivity of at least one test. Medical treatment included omeprazole, clarithromycin, and amoxicillin for 7 days followed by omeprazole for 3 weeks for duodenal ulcer and 5 weeks for gastric ulcer. Follow-up endoscopy with biopsies was performed 4 to 6 weeks after treatment cessation.
Results: Among 111 patients, 107 (96%) were infected by HP. The 4 patients non-infected by HP were treated by omeprazole alone and excluded from the study. Ulcer healed in 99 patients (93%). HP eradication was obtained in 102 patients (95%). The 8 patients with a non-healed ulcer and the 5 patients in whom HP eradication was not obtained at the first endoscopic control received additional medical treatment. Ultimately HP eradication and ulcer healing were obtained in all cases.
Conclusion: In a country in which prevalence of HP infection is high, acid-reduction surgery is unnecessary in case of perforated gastro-duodenal ulcer treated by simple surgical closure.
