Résumé |
Introduction |
La sédation est parfois recherchée chez les patients cancéreux en fin de vie afin de limiter leur perception de symptômes réfractaires aux traitements. Dans ce cadre, le midazolam est recommandé. Sa prescription est encadrée par des recommandations de bonnes pratiques. L’objectif principal de ce travail était de décrire les modalités de prescription du midazolam afin de les confronter aux recommandations et de tendre vers une meilleure utilisation du midazolam, en particulier dans la sédation.
Méthodes |
Nous avons rétrospectivement étudié les prescriptions de midazolam des patients décédés dans le service d’oncologie entre 2014 et 2015. Les taux de conformation aux recommandations étaient examinés.
Résultats |
Au total, 99 dossiers ont été analysés. Cinquante-trois patients avaient reçu du midazolam : 64,4 % pour symptômes réfractaires, 22 % pour anxiolyse, 10,2 % pour détresse respiratoire asphyxique. Lorsqu’elles étaient notées, les indications et modalités de titration étaient conformes aux recommandations dans 100 % des cas. Dans le cas de sédation pour symptômes réfractaires, les taux de conformation aux recommandations étaient de 76 % pour l’information des familles, 63 % pour l’information du patient, 34 % pour la réalisation d’une titration et 5 % pour la collégialité de la décision d’introduction.
Conclusion |
Si le manque de justification des prescriptions peut avoir entraîné une surestimation des sédations et une sous-estimation des titrations, les recommandations demeurent insuffisamment appliquées, notamment concernant la nécessité de collégialité de la décision de sédation. Une check-list est proposée pour optimiser les prescriptions. Notre travail souligne la nécessité d’une meilleure utilisation du midazolam en fin de vie.

Introduction |
At the end of life of cancer patients, sedation is sometimes needed, in order to palliate unbearable symptoms that other treatments fail to relieve. Midazolam is currently recommended for sedation and its prescription is guided by national guidelines. The aim of the present study was to evaluate the practices of midazolam prescription in a department of medical oncology and to compare the results with French national recommendations in order to improve the midazolam use in case of sedation.
Methods |
We conducted a retrospective survey studying midazolam prescriptions in patients who died between 2014 and 2015 in the medical oncology department. Compliance rates with recommendations were reviewed.
Results |
A total of 99 medical files were analyzed. Fifty-three patients received midazolam: 64.4% for refractory symptoms, 22% for anxiolyse, 10.2% for acute respiratory distress syndrome. When reported, the titration indications and modalities systematically conformed to guidelines. In case of sedation for refractory symptoms, adherence to guidelines was 76% regarding the family information, 63% regarding the patient information, 61% regarding the collegial nature of the initiation decision, 34% regarding the titration achievement and 5% regarding the pluridisciplinary nature of the initiation decision.
Conclusion |
Although limited justifications in midazolam prescriptions could have resulted in an overestimate of sedation and in an underestimate of titrations, current guideline are not enough applied in routine, especially regarding the necessity of a pluridisciplinary decision-making. A list of requirements before midazolam initiation is suggested, to increase the adherence to guidelines. Our results highlight the necessity of a better midazolam use in life-end sedations.
