Résumé |
Introduction |
Les thromboses artérielles et veineuses surviennent chez environ un patient sur cinq atteint de syndrome POEMS et touchent habituellement la macrocirculation.
Observation |
Nous décrivons le cas d’un homme de 67 ans atteint d’un syndrome POEMS qui consultait initialement devant des orteils bleus, associés à un phénomène de Raynaud, des paresthésies du pied gauche, une gynécomastie, des lésions cutanées évocatrices et des adénopathies. Le bilan cardiovasculaire ne mettait pas en évidence de cardiopathie emboligène, ni de lésion athéromateuse instable, ni de calcifications vasculaires. Le bilan morphologique trouvait des lésions ostéocondensantes multiples. La biologie mettait en évidence une gammapathie monoclonale lambda et le VEGF sérique était élevé à 2900pg/ml.
Conclusion |
Il s’agit à notre connaissance de la première description d’atteinte thrombotique microcirculatoire dans le syndrome POEMS, sans cause embolique ni calciphylaxie sous-jacente.

Introduction |
Arterial and venous thromboses occur in almost one in five patients with POEMS syndrome and usually in macrocirculation.
Case report |
We report a 67-year-old male with a POEMS syndrome who presented initially with a blue toe syndrome. He complained of Raynaud's syndrome and left foot paresthesia. Physical examination showed gynecomastia, lymphadenopathies and skin lesions. Cardiovascular investigations excluded atrial fibrillation, unstable atherosclerotic lesions and vascular calcifications. Imaging studies showed diffuse osteosclerotic lesions. Monoclonal protein with lambda light chain was discovered and serum level of VEGF was increased at 2900pg/ml.
Conclusion |
This is to our knowledge the first case of thrombotic microangiopathy in POEMS syndrome without embolic cause or calciphylaxis.
