La prise en charge de la polyarthrite rhumatoïde (PR) a connu ces dernières années une évolution remarquable avec l’avènement de thérapeutiques ciblées très efficaces. Ces traitements ont pu être développés grâce à une connaissance plus précise de la physiopathologie de la polyarthrite. Tous les patients ne répondent pas de manière suffisante ou échappent au cours du temps à ces traitements. Nous observons dans ce contexte une rotation des biothérapies et l’objet de cet article est de revoir les données publiées en cas de réponse inadéquate à un premier agent biologique. Le but est d’éclairer au mieux le clinicien pour définir la meilleure stratégie des agents biologiques au cours de la PR.

During the last years, the treatment strategy of rheumatoid arthritis (RA) patients has been largely improved due to the discovery of biological treatment. These agents have been developed based on a better understanding of the physiopathology of the disease. However, a proportion of the RA population does not benefit or escape to these therapies. Therefore, these patients switch from one agent to another one and the aim of this article is to review the data in this context of failure. The best strategy for a biological treatment in established RA will be discussed.
