Résumé |
Introduction |
L’endocardite infectieuse sur prothèse valvulaire est un challenge diagnostique et thérapeutique. La TEP/TDM au 18F-FDG apporte une aide considérable dans ce domaine et fait désormais partie des critères majeurs diagnostiques dans les recommandations de l’ESC de 2015. L’utilité potentielle de cet examen dans le suivi des patients reste à démontrer. L’objectif principal de l’étude était d’étudier l’impact pronostique en termes de récidive de la négativation ou de la persistance d’une positivité de la TEP/TDM de contrôle après traitement antibiotique.
Matériels et méthodes |
De janvier 2011 à avril 2016, étaient rétrospectivement inclus, à partir d’une cohorte, les patients présentant une endocardite infectieuse sur prothèse valvulaire certaine ou possible qui bénéficiaient d’une première TEP/TDM. Six à 10 semaines après la fin de l’antibiothérapie, les patients non opérés bénéficiaient d’une TEP/TDM de contrôle, en plus d’une échocardiographie et d’une consultation clinique.
Résultats |
Parmi les 276 patients porteurs d’une PV ayant bénéficié d’une TDM initiale pour suspicion d’endocardite infectieuse, 43 patients avec une TEP/TDM1 positive sur PV ont pu bénéficier d’une TEP/TDM de contrôle. Le suivi moyen était de 414jours. Trente-quatre patients présentaient une hyperfixation persistante sur prothèse valvulaire sur TEP/TDM de contrôle. Les cinq récidives concernaient des patients avec TEP/TDM de contrôle positive sur prothèse valvulaire.
Conclusion |
Il s’agit de la première étude à évaluer les apports de la TEP/TDM dans le suivi des endocardites infectieuses sur prothèses valvulaires non opérées traitées médicalement. La totalité des récidives survenait chez des patients ayant une fixation prothétique persistante après l’arrêt de l’antibiothérapie. Notre étude montrait également une tendance prometteuse sur le suivi des marqueurs indirects d’inflammation que sont les fixations spléniques et ostéomédullaires qui semblaient associés à un risque accru de récidive.

Introduction |
Prosthetic valve endocarditis is a diagnostic and therapeutic challenge; 18F-FDG PET/CT has seen increasing use and has been incorporated in the latest ESC endocarditis guidelines. Follow-up by PET/CT has never been studied. The aim was to study the use of PET/CT to help predicting recurrences after full antibiotic treatment.
Methods |
From January 2011 to April 2016, all patients presenting with definite or possible prosthetic valve endocarditis were included and had a PET/CT after admission. After discharge, non-operated patients had a second PET/CT from 6 to 10 weeks after the end of intravenous antibiotic treatment, in addition to clinical and echocardiographic follow-up.
Results |
Among 276 patients with prosthetic valve admitted for suspicion of infective endocarditis, 43 of them with an abnormal prosthetic uptake on the first PET/CT had a second PET/CT within a mean period of 67.3 days after the end of intravenous antibiotic treatment. The mean follow-up was 414 days. Five recurrences occurred among the 34 patients with persistent abnormal prosthetic uptake on second PET/CT, there was no recurrence among the nine patients who had no more abnormal prosthetic uptake.
Conclusion |
This is the first study to evaluate the potential benefits of PET/CT for medically treated PVE follow-up. All recurrences occurred among patients with persistent abnormal prosthetic uptake on second PET/CT. An unexpected finding was the potential of bone-marrow and splenic uptake evolution, which is underused in IE and showed an interesting potential in the prognosis of these patients.
